承气汤类方的演变特点与临床应用探析

摘要
《伤寒论》承气汤类方包括大、小承气汤,调胃承气汤,桃核承气汤,四者皆属泻下剂而有所不同.张仲景为阳明腑实"胃家实"创制大承气汤,据胃气强弱创立小承气汤、调胃承气汤,据瘀血创立桃核承气汤.历代医家应用承气汤类方时,根据临床实际情况,或原方应用,或增减药物组成,将泻下攻积与活血化瘀、清热解毒、调正护中等其他治法相结合,针对各种不同的病证特点创立和发展了诸多承气汤类方.总结承气汤类方的变化规律对临床治疗有重要的指导意义,值得深入挖掘和学习.
Abstract
Chengqi Decoction Associated Formulas in Treatise on Febrile Diseases include Da Chengqi Decoction,Xiao Chengqi Decoction,Tiaowei Chengqi Decoction and Taohe Chengqi Decoction.All of them are purgatives,but they are different.ZHANG Zhongjing created Da Chengqi Decoction for Yangming fu-organs'Weijiashi',and added and subtracted Xiao Chengqi Decoction and Tiaowei Chengqi Decoction according to the strength of stomach qi,and Taohe Chengqi Decoction according to blood stasis.In the application of Chengqi Decoction,doctors in past dynasties combined purgation method with other treatments methods such as activating blood circulation to remove blood stasis,clearing away heat and toxic materials,and regulating middle-aged care according to the clinical actual situation,or applying the original prescription or increasing or decreasing the drug components,and created and developed many Chengqi Decoction according to the characteristics of various diseases and syndromes.Summarizing the changing law of Chengqi Decoction has important guiding significance for clinical treatment,and it is worth digging and learning deeply.
