岐黄针疗法治疗肩袖修复术后中期疼痛临床疗效观察

摘要
目的:观察岐黄针疗法对关节镜下肩袖修复术(ARCR)后中期肩关节疼痛及功能的影响.方法:回顾性分析2021年1月至2022年12月在广州中医药大学第一附属医院进行ARCR的患者166例.最终纳入的60例患者根据术后6周是否进行岐黄针疗法治疗分成两组,对照组30例采用常规康复治疗,针疗组30例在对照组基础上配合岐黄针疗法治疗.分别于治疗前后记录两组患者的活动痛、夜间痛、静息痛视觉模拟评分(VAS),肩关节前屈、外展、后伸内旋活动度,美国加州大学肩关节评分(UCLA)和不良反应.结果:两组治疗后的各项指标均较前显著改善(P＜0.01),治疗后,针疗组活动痛、夜间痛VAS评分,前屈、外展活动度和UCLA评分较对照组改善更显著(P＜0.05),两组在治疗中未出现不良反应.结论:岐黄针疗法配合常规康复治疗能安全有效改善ARCR术后中期疼痛和活动度,促进肩关节功能康复.
Abstract
Objective:To observe the effect of Qihuang needle(QHN)therapy on mid-term shoulder pain and function after arthroscopy rotator cuff repair(ARCR).Methods:Retrospectively analyzed 166 patients who received ARCR in the First Affiliated Hospital of Guangzhou University of Chinese Medicine from January 2021 to December 2022.A total of 60 patients were finally included and were divided into 2 groups according to whether QHN therapy was performed 6 weeks after ARCR.The control group(30 cases)received conventional rehabilitation therapy,and the treatment group(30 cases)received QHN therapy based on the control group.The active,night,and rest pain visual analog scores(VAS),shoulder forward flexion,abduction,posterior extension internal rotation mobility,University of California-Los Angeles(UCLA)shoulder score,and adverse reactions were recorded before and after the treatment in both groups,respectively.Results:All measures in both groups were significantly better after treatment than before(P＜0.01).Active pain,night pain VAS scores,forward flexion,abduction mobility,and UCLA scores improved more significantly in the treatment group than in the control group after treatment(P＜0.05).No adverse effects were observed in either group during treatment.Conclusion:QHN therapy with conventional rehabilitation treatment can safely and effectively improve pain and mobility in the mid-term after ARCR,and promote shoulder functional rehabilitation.
