摘要
双相情感障碍是一类严重的精神障碍疾病,发病时会同现躁狂或轻躁狂和抑郁症状,或两者接替出现.目前其发病机制尚不清楚,我国的发病率不断上升,现代医学对其诊疗虽然取得了一定成效,但西药治疗不良反应较多、复发率高,中医学的整体观、辨证论治理论恰适用于双相情感障碍躁狂相和抑郁相完全相反的情况的辨别和诊治."土得木而达"理论是中医五行学说中对木土关系的理论探析,肝脾功能失调是双相情感障碍发病的重要致病因素,文章以"土得木而达"理论为指导,从肝脾关系探讨了双相情感障碍的病因病机以及相关治法,提出以肝脾为切入点,以调和肝脾、疏肝解郁、行气养血为治疗关键.
Abstract
Bipolar disorder(BD)is a serious mental disorder.When it occurs,mania or hypomania and depression symptoms will appear at the same time,or the two will appear alternately.At present,its pathogenesis is still unclear.The incidence rate in China is constantly increasing.Although modern medicine has achieved certain results in its diagnosis and treatment,Western medicine has large side effects and a high recurrence rate.The holistic view and syndrome differentiation theory of traditional Chinese medicine are suitable for the identification and diagnosis of BD,which is completely opposite to the manic and depressive phases.The theory of'earth being dredged by wood'is a theoretical exploration of the relationship between wood and earth in the theory of the five elements of traditional Chinese medicine.Liver and spleen dysfunction is an important pathogenic factor in the onset of BD.Guided by the theory of'earth being dredged by wood',this article explores the etiology and pathogenesis of BD from the perspective of the relationship between liver and spleen,as well as related treatment methods,and proposes to treat BD with liver and spleen as the starting point,and to harmonize the liver and spleen,relieve liver depression,and promote qi and nourish blood as the key.
