基于"玄府-络脉"理论探讨五期演变中"闭"在肿瘤恶性演进中的应用与辨治

摘要
基于"玄府-络脉"的理论内涵,阐释其与肿瘤恶性演进的关系.笔者认为"闭"是五期演变规律中重要的一环,也是肿瘤转向恶性预后的关键节点.提出"闭"的生物学基础为高渗透长滞留效应,临证将"闭"分为气闭、瘀闭及毒闭3种状态.其中,"气闭"为玄府失阖、络脉涸乏,是肿瘤恶性演进的病理基础,多以升陷汤理气升宣解郁;"瘀闭"为玄府不利、络脉瘀阻,是根本原因,治以理冲汤益气活血通络;"毒闭"为瘀毒停聚、络道亢变,作为转折核心,故以扶正活血祛毒立意,结合不同病势进行加减配伍.三闭全流程调控,使玄府开阖有度,络脉畅达盈满,冀以为临床诊治提供新思路.
Abstract
Based on the theoretical connotation of the theory of'Xuanfu-choroid',the relationship between'Xuanfu-choroid'and malignant evolution of tumor was explained.Authors think that'closure'is an important link in the evolution of the five stage,and is also a key node for the prognosis of malignant transformation.It is suggested that the biological basis of'closure'is EPR effect.Clinically,'closure'can be divided into three states:qi closure,yu closure and toxic closure.Among them,'qi closure is Xuanfu has collapsed,and Luomai has dried up,that are the pathological basis of malignant progression of tumors.Shengxian Decoction is usually used to regulate qi rise and relieve depression,'yu closure'is Xuanfu is not good,and blockage of complex veins,that is the root cause.In order to cure Lichong Decoction and promote qi and blood circulation,'toxic closure'stops stasis and toxin accumulation,and hyperactivity of collateral and Dao,that is taken as the core of turning point.Therefore,with the intention of promoting the right blood circulation and detoxification the combination of different disease conditions was added and subtraction.Three close the whole process of regulation,so that the Xuanfu opening and closing degree,the complex smooth full,hoping to provide new ideas for clinical diagnosis and treatment.
