目的:基于代谢组学探究呼吸导引康复技术治疗稳定期慢性阻塞性肺疾病(COPD)的作用机制.方法:60例稳定期COPD患者作为患者组,随机分为试验组和对照组,同时招募30名健康志愿者为健康组.对照组给予西医常规治疗,试验组在此基础上联合呼吸导引康复技术治疗,疗程12周.采集治疗前后及健康组血清,通过超高效液相色谱质谱联用技术筛选血清差异代谢物,采用MetaboAnalysis 5.0软件进行通路分析.结果:健康组与患者组比较,鉴定出31个显著性差异代谢物(P<0.05),对照组和试验组治疗前后分别有9、23个代谢物显著回调(P<0.05);与对照组治疗后比较,试验组治疗后[PC(18∶1(9Z)/P-18∶1(11Z))、PC(16∶1(9Z)/P-18∶0)、LysoPC(24∶0)、LysoPC(22∶0)、LysoPC(20∶0/0∶0)]显著回调(P<0.01,P<0.05),代谢物主要富集于甘油磷脂代谢通路.结论:呼吸导引康复技术可改善COPD稳定期患者的代谢紊乱,可能主要通过调节甘油磷脂代谢发挥作用,尚需进一步开展验证研究.
Objective:To explore the mechanism of respiratory guided rehabilitation technique in the treatment of stable phase chronic obstructive pulmonary disease(COPD)based on metabolomics.Methods:A total of 60 patients with stable phase COPD were as patient group,and were randomly divided into treatment group and control group,and 30 healthy controls were recruited as healthy group.The control group was treated with routine western medicine,and the treatment group was treated with respiratory guidance rehabilitation technique for 12 weeks.The serum was collected before and after treatment and in the healthy group.The differential metabolites in serum were screened by UPLC-MS/MS technique,and the pathway was analyzed by MetaboAnalysis 5.0.Results:There were significant differences between the healthy group and the patient group,and 31 differential metabolites were identified(P<0.05).After treatment,9 metabolites in the control group and 23 metabolites in the treatment group could be significantly recalled(P<0.05).Compared with the control group after treatment,the treatment group after treatment showed significant pullback[PC(18∶1(9Z)/P-18∶1(11Z)),PC(16∶1(9Z)/P-18∶0),LysoPC(24∶0),LysoPC(22∶0),LysoPC(20∶0/0∶0)](P<0.01,P<0.05).The metabolites were mainly enriched in the glycerol phospholipid metabolic pathway.Conclusion:Respiratory guidance rehabilitation technique can improve the metabolic disorder of patients with stable phase COPD,which may play a role mainly by regulating the metabolism of glycerol phosphorlipid,further verification research is needed.
