摘要
目的:探讨甘油三酯葡萄糖乘积(TyG)指数与2型糖尿病肾病内热证之间的相关性.方法:共纳入227例2型糖尿病肾病患者,内热证152例,非内热证75例,对两组的TyG指数及肾功能进行统计,并对糖尿病肾病(DKD)内热证及TyG等指标采用二项Logistic回归分析及相关性分析.结果:DKD患者内热证的发生率在66.96％(152/227),内热证患者的TyG指数、甘油三酯(TG)、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、血肌酐(Scr)均较非内热证患者显著升高(P＜0.01,P＜0.05),而估算肾小球滤过率(eGFR)较非内热证显著降低(P＜0.01).二项Logistic回归分析显示:DKD内热证发生与TyG和eGFR独立相关(P＜0.01).相关性分析结果显示:TyG与eGFR、内热证呈显著性正相关(P＜0.05,P＜0.01),与Scr呈显著性负相关(P＜0.05).结论:TyG、DKD内热证、肾功能之间均存在相关,内热证患者肾功能损伤更明显、TyG指数升高更显著,提示TyG是DKD内热证发生的独立危险因素,亦是DKD进展的重要标志物.
Abstract
Objective:To explore the correlation between the triglyceride glucose(TyG)index and internal heat syndrome in patients with type 2 diabetic kidney disease.Methods:A total of 227 patients with type 2 diabetic kidney disease were included,of which 152 had internal heat syndrome and 75 did not.The TyG index and renal function laboratory indicators were statistically analyzed between the two groups.Logistic regression analysis and correlation analysis were used to analyze the internal heat syndrome in diabetic kidney disease(DKD)and TyG index.Results:The incidence of internal heat syndrome in DKD patients was 66.96％(152/227).Compared with patients without internal heat syndrome,patients with internal heat syndrome had significantly higher TyG index,TG,FBG,HbA1c,and Scr(P＜0.01,P＜0.05),and significantly lower eGFR(P＜0.01).The Logistic regression analysis showed that the occurrence of internal heat syndrome in DKD was independently correlated with TyG and eGFR(P＜0.01).The correlation analysis showed that TyG was positively correlated with eGFR and internal heat syndrome(P＜0.05,P＜0.01),and negatively correlated with Scr(P＜0.05).Conclusion:There is a correlation between TyG,internal heat syndrome in DKD,and renal function.Patients with internal heat syndrome have more significant kidney damage and a more significant increase in the TyG index,suggesting that TyG is an independent risk factor for the occurrence of internal heat syndrome in DKD and an important indicator of DKD progression.
