导赤散合清营汤加减治疗寻常型银屑病血热证的临床疗效观察

摘要
目的:观察导赤散合清营汤加减治疗寻常型银屑病血热证的临床疗效.方法:收集2021年6月至2022年12月就诊于山东中医药大学附属医院的寻常型银屑病血热证患者178例,随机分为观察组和对照组,观察组予导赤散合清营汤加减口服联合卡泊三醇软膏外用,对照组予消银颗粒口服联合卡泊三醇软膏外用,均治疗8周.分别观察和记录两组临床疗效指标、生活质量指标、病情指标及不良反应情况.结果:治疗8周后,观察组总有效率[84.62％(66/78)]显著高于对照组[67.12％(49/73)](P＜0.01),观察组银屑病皮损面积严重程度指数(PASI)、体表受累面积(BSA)、视觉模拟评分(VAS)和皮肤病生活质量指数(DLQI)评分均较治疗前显著降低(P＜0.01),对照组PASI、BSA和DLQI评分均较治疗前显著降低(P＜0.01,P＜0.05),观察组PASI、BSA、VAS和DLQI评分均显著低于对照组(P＜0.01).两组不良反应发生率比较,差异无统计学意义.结论:导赤散合清营汤加减治疗寻常型银屑病血热证疗效显著,且不良反应发生率低,是值得推广应用于临床的有效方剂.
Abstract
Objective:To observe the clinical efficacy of Daochi Powder and Qingying Decoction in the treatment of psoriasis vulgaris with blood heat syndrome.Methods:A total of 178 psoriasis vulgaris patients with blood heat syndrome in Affiliated Hospital of Shandong University of Traditional Chinese Medicine from June 2021 to December 2022 were randomly divided into the observation group and the control group.The observation group was treated with oral medicine Daochi Powder and Qingying Decoction and Calcipotriol ointment for external use.The control group was treated with oral medicine Xiaoyin Granule and Calcipotriol Ointment for external use.Both groups were treated for 8 weeks.Efficacy indexes,quality of life indexes,disease indexes and adverse effects of the two groups were observed and compared.Results:After 8 weeks of treatment,the total effective rate in the observation group[84.62％(66/78)]was better than that in the control group[67.12％(49/73)](P＜0.01).The scores of psoriasis area and severity index(PASI),body surface area(BSA),visual analogue scale(VAS)and dermatology quality of life index(DLQI)in the observation group were significantly lower than those before treatment(P＜0.01),the scores of PASI,BSA and DLQI in the control group were significantly lower than those before treatment(P＜0.01,P＜0.05),the PASI,BSA,VAS and DLQI of the observation group was significantly lower than that in the control group(P＜0.01).There was no difference in the incidence of adverse reactions.Conclusion:Daochi Powder and Qingying Decoction has significant effect on blood heat syndrome of psoriasis vulgaris,and the adverse reaction rate is low,it is worth promoting the application of effective prescription in clinic.
