藏族患者万古霉素血药谷浓度与疗效和肾毒性的相关性研究

目的:探讨万古霉素血药谷浓度监测与藏族患者治疗效果、肾毒性之间的相关性,为藏族患者万古霉素个体化用药提供参考。方法:连续纳入2020年4月至2022年9月西藏自治区人民政府驻成都办事处医院收治使用万古霉素并接受治疗药物监测的藏族患者,根据谷浓度监测结果进行分组,分别比较感染治疗效果和肾毒性发生风险。结果:共纳入88例患者,其中有治疗效果评价结果的病例88例,有肾毒性评价结果的病例81例。单因素和多因素分析结果均提示在藏族患者中,谷浓度是否低于10mg·L-1对于感染治疗效果的影响无统计学意义,而谷浓度是否高于20 mg·L-1对于肾毒性发生风险的影响也无统计学意义。结论:当前常用的万古霉素血药谷浓度范围(10~20mg·L-1)可能并不适于藏族人群的血药浓度监测,仍需开展该人群的群体药动学研究以进一步探寻适合的万古霉素血药谷浓度监测范围。
Analysis of the correlation between serum through concentration of vancomycin and its efficacy and neph-rotoxicity in Tibetan patients
OBJECTIVE To investigate the correlation between the serum trough concentration of vancomycin and its thera-peutic effect and nephrotoxicity in Tibetan patients with infection,so as to provide reference for the individualized medication of vancomycin in Tibetan patients.METHODS Tibetan patients who were treated with vancomycin and received treatment drug monitoring in Hospital of Chengdu Office of People's Government of Tibetan Autonomous Region from April 2020 to September 2022 were consecutively enrolled.The patients were divided into groups according to the serum trough concentration monitoring results,and the treatment effect of infection and the risk of nephrotoxicity were compared.RESULTS A total of 88 patients were enrolled.Totally 88 cases had treatment effect evaluation results,and renal toxicity evaluation results were available in 81 cases.The results of univariate and multivariate analyses showed that serum trough concentration lower than 10 mg·L-1 had no statisti-cally significant effect on the efficacy of infection treatment,and serum trough concentration higher than 20 mg·L-1 had no statisti-cally significant effect on the risk of nephrotoxicity in Tibetan patients.CONCLUSION The current serum trough concentration range of vancomycin(10-20 mg·L-1)may not be suitable for monitoring the blood concentration of vancomycin in Tibetan popu-lation.Population pharmacokinetics studies in Tibetan population are needed to further explore the suitable trough concentration range of vancomycin.
