罗库溴铵致过敏性休克48例文献分析

摘要
目的:分析罗库溴铵致过敏性休克的临床表现和特点,为临床安全用药提供参考.方法:检索PubMed、EmBase、Web of Science、万方、知网、维普数据库自建库至2022年12月关于罗库溴铵致过敏性休克的中英文案例报道,并对文献进行统计分析.结果:共纳入案例报道43篇,合计48例患者;患者年龄3～81岁,中位数年龄33.0岁,其中青年(18～44岁)和老年(≥60岁)占比较高(32例,66.7％);女性(26例)多于男性(22例);除25例过敏性休克的时间记录不详外,其余23例均发生在30 min以内;累及的器官系统损害涉及循环系统(100例次,51.5％)、呼吸系统(49例次,25.3％)、皮肤及其附件(45例次,23.2％);抢救措施包括立即停药、吸氧、给予血管活性药物和糖皮质激素等抗休克治疗,最终46例抢救成功,2例死亡.结论:罗库溴铵致过敏性休克可导致致命的后果,临床应加强用药监护,做到及时处理,确保用药安全.
Abstract
OBJECTIVE To analyze the clinical presentations and characteristics of anaphylactic shock induced by rocuronium,and to provide reference for the safe clinical medication.METHODS PubMed,EmBase,Web of Science,Wan-fang,CNKI and VIP databases were searched for case reports in both Chinese and English involving anaphylactic shock induced by rocuronium from inception to December,2022.The literature were summarized systematically.RESULTS A total of 43 case reports involving 48 patients were included.The age of the patients ranged from 3 to 81 years old,with a median age of 33.0 years.Young(18-44 years)patients and old(≥60 years)patients accounted for a relatively high proportion(32 cases,66.7％),and there were more females(26 cases)than males(22 cases).The duration of anaphylactic shock(23 cases)occurred within 30 minutes except the records of 25 cases were unknown.The damages involved circulatory system(100 cases,51.5％),respiratory system(49 cases,25.3％),and skin and its appendage(45 cases,23.2％).The first-aid measures included discontinuation of medicine immediately,oxygen uptake,and administration of vasoactive drugs and glucocorticoids,etc.Finally,46 cases were successfully rescued,and 2 cases died.CONCLUSION Anaphylactic shock induced by rocuronium can lead to fatal outcomes.Drug monitoring should be strengthened in clinic to achieve timely treatment and ensure the clinical medication safety.
