Octreotide in the treatment of high flow and complex chylous fistula after neck dissection for head and neck cancer:a case report
OBJECTIVE To analyze a case of high flow complex chylous fistula with octreotide treatment and provide refer-ence for the management of chylous fistula following neck dissection for head and neck cancer.METHODS The clinical manifes-tations,causes,initial and post-recurrence treatment in a case of high flow and complex chylous fistula following neck dissection for head and neck cancer were analyzed in combination with related literature.RESULTS The patient was discharged from hospi-tal after treatment with total parenteral nutrition,pressure dressing and negative pressure wound treatment when first diagnosed with chylous fistula.Although the conventional treatment of total parenteral nutrition and pressure dressing was given from the beginning of admission when chylous fistula recurred,it was controlled after 10-day octreotide treatment,therefore the successful treatment of chylous fistula was contributed to octreotide.CONCLUSION Octreotide has a good prospect in the treatment of high flow complex chylous fistula following neck dissection for head and neck cancer,however,other confounding factors may be involved.Our findings need to be verified in large-scale,randomized,multi-center trials.
