Hollow fiber centrifugal ultrafiltration-high performance liquid chromatography for the determination of trough concentration of biapenem in intensive care unit patients
OBJECTIVE To establish a rapid,simple and low-cost method for determining the plasma concentration of biap-enem and measuring the trough concentration in intensive care unit(ICU)patients.METHODS A method for measuring the concentration of biapenem in human plasma was established and validated.From March 2023 to August 2023,8 ICU patients on a therapy of biapenem were prospectively recruited and their trough concentrations measured.RESULTS The standard curve of biapenem was:y=0.179 6x+0.019 3(r=0.999 9),0.2-50 μg·mL-1.Linear relationship was decent with a lower limit of quan-tification at 0.2 μg·mL-1.The absolute recoveries of low,medium and high concentrations(0.5,5,40 μg·mL-1)averaged 97.40%,99.43%and 93.58%and the method recoveries reached 100.55%,94.81%and 97.41%.For the above three concen-trations,intraday relative standard deviation(RSD)value was<5.0%and the concentration range of biapenem was 0.51-1.79 μg·mL-1.The results of blood concentration determination in ICU patients have demonstrated that biapenem has a decent efficacy for Enterobacteriace.And its effect may not be ideal for infections caused by Proteus mirabilis,Klebsiella pneumoniae or Acinetobacter baumannii.CONCLUSION This method is simple,accurate,reproducible and inexpensive.It may be used for routine determination of biapenem biological samples.Therapeutic drug monitoring should be performed for ICU patients so that dosing regimen may be adjusted in time.
