目的:系统评价奥妥珠单抗联合化疗治疗滤泡性淋巴瘤的药物经济学价值以及经济性研究中的模型运用，为卫生政策的制订和临床决策提供参考依据。方法:检索PubMed、ScienceDirect、Cochrane系统评价数据库、Med line、Embase、Web of Science、中国知网和万方数据知识服务平台等数据库，时间段为各自建库至2023年9月，并辅以手工检索和文献追溯等方法，纳入公开发表的奥妥珠单抗的药物经济学相关文献。根据纳入标准筛选出的文献，按照《卫生经济学评价报告标准共识》(2022版)量表对文章进行质量评价，并对纳入文献中使用的模型以及得到的研究结果进行分析和总结。结果:共纳入文献7篇，从模型来看，有5篇使用了马尔科夫模型，2篇使用了分区生存模型;从方法来看，7篇均采用成本-效果分析，其中2篇同时使用了成本-效用分析。经统计，奥妥珠单抗联合化疗的增量成本效果比值均在各国意愿支付值之内。结论:本研究显示奥妥珠单抗联合化疗比利妥昔单抗联合化疗更具有经济性，这为我国卫生决策、医院药品遴选和医保准入进一步提供证据。
Pharmacoeconomic systematic evaluation of treating follicular lymphoma with obinutuzumab plus chemotherapy
OBJECTIVE To systematically evaluate the pharmacoeconomic value of obinutuzumab plus chemotherapy for follicular lymphoma(FL)and explore the application of models for economic studies to provide references for health policy draft-ing and clinical decision-making.METHODS The databases of PubMed,ScienceDirect,Cochrane Database of Systematic Reviews,Medline,Embase,Web of Science,CNKI and Wanfang Data Knowledge Platform were searched from their inceptions until September 2023.Manual retrieval and literature tracing were also employed for retrieving pharmacoeconomic publications related to obinutuzumab.The qualities of eligible papers were evaluated by the scale of"Consensus on Health Economics Evalua-tion Report Standards"(Edition 2022).And inclusion models and searching results were analyzed.RESUITS Among 7 papers,5 papers used Markov model and 2 papers partitioned survival model.As for methodology,all 7 papers used cost-effectiveness analysis and 2 of them cost-utility analysis.Statistically,incremental cost-effectiveness ratios(ICERs)for combined chemotherapy with obinutuzumab were within the willingness-to-pay values for each country.CONCLUSION Combined chemo-therapy with obinutuzumab is more economical than combined chemotherapy with rituximab.It provides further evidence for health policy making,hospital drug procuring and health insurance access in China.
