黄连温胆汤治疗大肠湿热型溃疡性结肠炎的疗效观察

摘要
目的:探讨黄连温胆汤治疗大肠湿热型溃疡性结肠炎的临床疗效.方法:选取 2021 年 10 月至 2023 年 2 月广州中医药大学附属中山中医院收治的大肠湿热型溃疡性结肠炎患者 60 例,按照随机数字表法分为对照组和观察组,每组 30 例.对照组患者口服美沙拉嗪肠溶片,观察组患者口服美沙拉嗪肠溶片和黄连温胆汤,疗程 30 d.比较两组患者治疗前后白细胞计数(WBC)、C反应蛋白(CRP)、红细胞沉降率(ESR)和白细胞介素 6(IL-6)等感染指标变化,进行肠镜Mayo评分和溃疡性结肠炎内镜严重程度指数(UCEIS)评分,记录治疗过程中腹泻、脓血便和腹痛等症状消失时间并进行统计分析.结果:(1)治疗后,观察组患者的总有效率为 83.33%(25/30),明显高于对照组的 66.67%(20/30),差异有统计学意义(P<0.05).(2)治疗后,两组患者WBC、CRP、ESR和IL-6水平均较治疗前明显降低,差异均有统计学意义(P<0.05);两组患者治疗后各项感染指标比较,差异无统计学意义(P>0.05).(3)观察组患者腹泻、脓血便和腹痛等症状消失时间明显短于对照组,差异均有统计学意义(P<0.05).(4)治疗后,观察组 25 例患者肠黏膜损伤得到明显修复,对照组 20 例患者肠黏膜损伤得到明显修复;观察组患者的Mayo评分和UCEIS评分明显低于对照组,差异均有统计学意义(P<0.05).结论:黄连温胆汤能有效改善大肠湿热型溃疡性结肠炎患者的临床症状,促进肠黏膜修复,提高患者的生活质量,且无明显不良反应,对大肠湿热型溃疡性结肠炎有一定疗效.
Abstract
OBJECTIVE:To probe into the clinical efficacy of Huanglian Wendan decoction in the treatment of ulcerative colitis of large intestine with damp-heat type.METHODS:Totally 60 patients with ulcerative colitis of large intestine with damp-heat type admitted into Zhongshan Hospital of Traditional Chinese Medicine Affiliated to Guangzhou University of Traditional Chinese Medicine from Oct.2021 to Feb.2023 were extracted to be divided into the control group and the observation group via the random number table method,with 30 cases in each group.The control group was treated with Mesalazine enteric-coated tablets alone,while the observation group received Mesalazine enteric-coated tablets combined with Huanglian Wendan decoction.The treatment course was 30 d.Changes of white blood cells(WBC),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),interleukin 6(IL-6)and other indicators of infection were compared before and after treatment in two groups.Colonoscopy Mayo score and ulcerative colitis endoscopic severity index(UCEIS)score were performed to record the disappearance time of diarrhea,bloody stool and abdominal pain during treatment and statistical analysis was performed.RESULTS:(1)After treatment,the overall effective rate of observation group was 83.33%(25/30),significantly higher than that of control group 66.67%(20/30),the difference was statistically significant(P<0.05).(2)After treatment,the levels of WBC,CRP,ESR and IL-6 in both groups were significantly lower than those before treatment,with statistically significant difference(P<0.05).There was no significant difference in infection indicators between two groups after treatment(P>0.05).(3)The disappearance time of diarrhea,pus and blood stools,and abdominal pain in the control group was significantly longer than that in the observation group,the difference was statistically significant(P<0.05).(4)After treatment,intestinal mucosal damage was significantly repaired in 25 patients in the observation group and 20 patients in the control group.Mayo and UCEIS scores in the observation group were significantly lower than those in the control group,with statistically significant difference(P<0.05).CONCLUSIONS:Huanglian Wendan decoction can effectively improve the clinical symptoms of ulcerative colitis of large intestine with damp-heat type,promote intestinal mucosal repair,and improve the quality of life of patients,and has no obvious adverse drug reactions.
