苦参凝胶治疗外阴阴道假丝酵母菌病(湿热下注证)的药物经济学评价
Pharmacoeconomic Evaluation of Sophora Flavescens Gel in the Treatment of Vulvovaginal Candidiasis(Dampness-Heat Syndrome)

摘要
目的:评价苦参凝胶治疗外阴阴道假丝酵母菌病(VVC,湿热下注证)的经济性.方法:从卫生体系角度出发,基于 1 项已发表的苦参凝胶治疗VVC的随机对照试验研究,以该研究中的假丝酵母菌转阴率作为效果参数,从米内网数据库获得成本参数,应用TreeAge Pro软件对苦参凝胶治疗VVC(湿热下注证)进行成本-效果分析.结果:决策树模型模拟结果显示,患者用药治疗 14 d后,苦参凝胶联合克霉唑阴道片的成本为 123.28 元,外阴阴道假丝酵母菌转阴率为 0.93;单纯使用克霉唑阴道片的成本为86.56 元,假丝酵母菌转阴率为0.71.与单纯使用克霉唑阴道片相比,苦参凝胶联合克霉唑阴道片的成本较高,疗效也更好,增量成本-效果比为 166.91 元,约为 2020 年我国人均可支配收入(32 189 元)的 0.52%,敏感性分析显示结果较为稳健.结论:基于2020 年我国人均可支配收入作为患者意愿支付阈值的假设,苦参凝胶联合克霉唑阴道片治疗VVC比单纯使用克霉唑阴道片更具有经济性.本研究的开展有益于临床医师合理用药或选用经济性更优的方案,同时对于卫生医疗资源配置方面可以提供优化的参考依据.
Abstract
OBJECTIVE:To evaluate the economics of Sophora flavescens gel in the treatment of vulvovaginal Candidiasis(VVC,dampness-heat syndrome).METHODS:From the perspective of health system,based on a published randomized controlled trial study on the treatment of VVC by Sophora flavescens gel,the negative conversion rate of candida in this study was used as the effect parameter,the cost parameters were obtained from the database of Menet.com,and the cost-effectiveness analysis was conducted by using TreeAge Pro software for Sophora flavescens gel in the treatment of VVC.RESULTS:Simulation results of decision tree model showed that after 14 d of treatment,the cost of Sophora flavescens gel combined with Clotrimazole vaginal tablet was 123.28 yuan,and the conversion rate of candida in vulva vagina was 0.93.The cost of using Clotrimazole vaginal tablet alone was 86.56 yuan,and the negative conversion rate of candida was 0.71.Compared with Clotrimazole vaginal tablet alone,Sophora flavescens gel combined with Clotrimazole vaginal tablet had higher cost and better efficacy,with an incremental cost-effectiveness ratio of 166.91 yuan,about 0.52%of per capita disposable income in China in 2020(32 189 yuan),and sensitivity analysis showed that the results were relatively robust.CONCLUSIONS:Based on the assumption that per capita disposable income is the patient's willing to pay threshold in China in 2020,the treatment regimen of Sophora flavescens gel combined with Clotrimazole vaginal tablet for VVC is more economical than Clotrimazole vaginal tablet alone.The development of this study is beneficial for clinicians to rationally use drugs or select more economical regimens,and can provide optimized reference for the allocation of health care resources.
