沙参麦冬汤加减方联合阿奇霉素治疗小儿支原体肺炎的疗效及安全性的系统评价与Meta分析

摘要
目的:系统评价沙参麦冬汤加减方联合阿奇霉素治疗小儿支原体肺炎的临床疗效及安全性.方法:计算机检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库、PubMed、Embase、Web of Science等中英文数据库中沙参麦冬汤加减方联合阿奇霉素治疗小儿支原体肺炎的临床随机对照试验(观察组患者采用沙参麦冬汤加减方联合阿奇霉素治疗,对照组患者采用阿奇霉素治疗),时间为建库至 2023 年 10 月,筛选文献、提取数据后,对纳入的研究进行Meta分析.结果:共纳入 13 项研究,总病例数为 1144 例.Meta分析结果显示,相比对照组,沙参麦冬汤加减方联合阿奇霉素能够有效提高小儿支原体肺炎患者的临床总有效率(OR=5.65,95%CI=3.56～8.97,P<0.00001),缩短退热时间(MD=-2.88,95%CI=-5.11～-0.64,P=0.01)、咳嗽消失时间(MD=-2.78,95%CI=-4.63～-0.94,P=0.003)和肺部啰音消失时间(MD=-2.91,95%CI=-4.27～-1.54,P<0.0001),降低复发率(OR=0.21,95%CI=0.07～0.63,P=0.005),同时可降低不良反应发生率(OR=0.21,95%CI=0.12～0.37,P<0.00001),差异均有统计学意义.结论:中药沙参麦冬汤加减方联合阿奇霉素治疗小儿支原体肺炎具有一定的优势,可提高临床疗效,降低不良反应发生率,但仍需高质量、大样本、多中心的随机对照试验加以证实.
Abstract
OBJECTIVE:To systematically evaluate the clinical efficacy and safety of modified Shashen Maidong decoction combined with azithromycin in the treatment of Mycoplasma pneumonia in children.METHODS:CNKI,Wanfang Data,VIP,CBM,PubMed,Embase,Web of Science and other Chinese and English databases were retrieved to collect randomized controlled trial of modified Shashen Maidong decoction combined with azithromycin in the treatment of Mycoplasma pneumonia in children(observation group was treated with modified Shashen Maidong decoction combined with azithromycin,while control group received azithromycin),the retrieval time was from the database establishment to Oct.2023.After literature screening and data extraction,Meta-analysis was conducted on the included studies.RESULTS:A total of 13 studies including 1144 patients were enrolled.Meta-analysis showed that compared with the control group,modified Shashen Maidong decoction combined with azithromycin could effectively improve the total clinical effective rate of children with Mycoplasma pneumonia(OR=5.65,95%CI=3.56-8.97,P<0.00001)and shorten fever remission time(MD=-2.88,95%CI=-5.11--0.64,P=0.01),cough disappearance time(MD=-2.78,95%CI=-4.63--0.94,P=0.003)and lung rale disappearance time(MD=-2.91,95%CI=-4.27--1.54,P<0.0001),decreased the recurrence rate(OR=0.21,95%CI=0.07-0.63,P=0.005),and the incidence of adverse drug reactions(OR=0.21,95%CI=0.12-0.37,P<0.00001),the differences were statistically significant.CONCLUSIONS:Shashen Maiodong decoction combined with azithromycin has certain advantages in the treatment of Mycoplasma pneumonia in children,which can improve clinical efficacy and decrease incidence of adverse drug reactions,but the conclusion still needs to be confirmed by high-quality,large sample and multi-center randomized controlled trials.
