Clinical characteristics and prognostic factors among AIDS patients with Talaromyces marneffei infection
Objective To explore the clinical characteristics and prognostic factors of AIDS complicated with Talaromyces marneffei(TM)infection.Methods A total of 85 patients with AIDS complicated with TM infection admitted to our hospi-tal from January 2017 to December 2021 were selected.According to the prognosis,they were divided into survival group and death group.The clinical and laboratory indexes of the two groups were compared and analyzed.Prognostic factors were analyzed by logistic regression.Results The mortality rate of AIDS patients with TM infection was 18.8%.The most com-mon symptoms and signs were fever(94.1%),cough(54.1%),emaciation(54.1%),skin lesions(34.1%),hepato-splenomegaly(38.8%)and lymphadenopathy(80.0%).The most common laboratory abnormalities were anaemia(78.8%),thrombocytopenia(45.9%),and elevated aspartate aminotransferase(70.6%)and lactate dehydrogenase(80.0%)levels.Multivariate logistic regression analysis showed variables including hemoglobin(OR=0.922;95%CI 0.858,0.991),urea nitrogen(OR=1.612;95%CI 1.084,2.399),respiratory failure(OR=348.538;95%CI 5.374,22606.322)were prognostic factors of AIDS patients with TM infection.Conclusion Respiratory failure,decreased hemo-globin and increased urea nitrogen are risk factors for poor prognosis in AIDS patients with TM infection.
