免疫球蛋白M和糖蛋白96表达与鼻咽癌放疗敏感性的关系

摘要
目的 研究免疫球蛋白M(immunoglobulinM,IgM)和糖蛋白96(glycoprotein 96,gp96)在鼻咽癌(nasopharyngeal carcinoma,NPC)中表达与放疗敏感性的关系.方法 按照世界卫生组织关于实体肿瘤放射治疗敏感性评估标准选取60例NPC放疗前组织标本,其中30例为放疗敏感组,30例为放疗耐受组;并取30例同期鼻咽部慢性炎症作为对照;另选取同期放射治疗3-6个月后颈部残留淋巴结转移病灶并行颈部淋巴结清扫者9例;采用免疫组化SP法检测IgM和gp96的表达,比较分析IgM和gp96表达与NPC放疗敏感性的关系.结果 IgM和gp96在NPC组织中均显著高表达.IgM在NPC放疗耐受组的表达率(80.0%)显著高于放疗敏感组(36.7%)(P<0.05);gp96在NPC放疗耐受组的表达率(83.3%)也显著高于放疗敏感组(46.7%)(P<0.05);IgM和gp96在NPC组织中的表达呈正相关性(r=0.443,P<0.05).IgM与gp96在NPC放疗后颈部淋巴结转移残留组织中高表达率分别为88.9%和77.8%.结论 IgM和gp96在NPC中高表达,且其表达水平与NPC放疗敏感性呈负相关,提示它们可能作为预测NPC放疗敏感性的指标;IgM和gp96在NPC中表达呈正相关,同时检测两个指标可能会提高预测NPC放疗敏感性的准确度,以便制定更为有效的治疗方案.
Abstract
Objective To study the relationship between the expression of ImmunoglobulinM (IgM) and glycoprotein 96(gp96) in nasopharyngeal carcinoma (NPC) and sensitivity to radiotherapy. Methods According to the WHO on solid tumor radiosensitivity evaluation criteria, 60 cases of NPC tissue specimens before radiotherapy, including 30 cases of radiation sensitive group, 30 cases of radiation resistance group, were chosen, and 30 cases of chronic nasopharngitits as control group; 9 cases of residual cervical lymph node were also selected in cervical lymph node dissection after radiotherapy 3-6 months. The expression of IgM and gp96 were detected by immunohistochemistry SP method, and the relationship between the expression of IgM and gp96 and the radiosensitivity of NPC was analyzed. Results IgM and gp96 were both over-expressed in NPC tissues. The expression of IgM in radio-resistant group (80.0%) was significantly higher than radio-sensitive group (36.7%) (P<0.05). Also, the expression of gp96 in radio-resistant group (83.3%) was significantly higher than radio-sensitive group (46.7%)(P<0.05). The expression of IgM and gp96 had positive correlation in NPC(r=0.443, P<0.05). The high expression of IgM and gp96 in residual metastatic neck lymph node after radiotherapy were 88.9%and 77.8% separately. Conclusions IgM and gp96 were both over-expressed in NPC. Both of them had negative correlation with radiosensitivity of NPC, which could be a useful predictor of radiosensitivity in NPC. The expression of IgM and gp96 had positive correlation in NPC. Detecting the expression of IgM and gp96 can more accurately predict the radiosensitivity of NPC.
