Application of the submental island flap in hypopharyngeal carcinoma reserved laryngeal function surgery
Objective To explore the effective application of the submental island flap in hypopharyngeal carcinoma reserved laryngeal function surgery.Methods A retrospective study of clinical data of 30 cases with hypopharyngeal carcinoma applicated with submental island flap reserved laryngeal function surgery were retrospective studied.Results Those 30 cases of the hypopharyngeal carcinoma were treated by tumors resection reserved laryngeal function and repaired with submental island flap repairment in the same time.7 cases were treated with radiotherapy alonely and 18cases were treated with radiotherapy and chemotherapy postoperatively.Those follow-up results were recorded.Of 30 cases of the hypopharyngeal carcinoma,all the submental flaps survived postoperatively,including 4 cases delayed survived because of the partial vascular blockage resulted in partial necrosis of those flaps.No pharyngeal leakage occurred.The average time of oral intake resumed was 34+/-11 days postoperatively.The average time of tracheal catheter extraction was 93+/-21 days after surgery.The 2-,3-and 5-year survival rates were 75.3%,50.8%and 43.7%,respectively.Local recurrence with lymph node metastasis in 8 cases,lung metastasis in 6 cases,esophageal cancer in 4 cases.?There were 16 cases of survivors without tumor,7 cases of survivors with tumor and and 3 cases of death.Conclusion The submental island flap has many advantages in repairing the defect of hypopharyngeal tumor resection,such as large amount of skin tissue,rich blood transport,strong anti-infection ability,and the skin incision of the donor site closed directly in most cases.
