单侧完全性唇腭裂患者咽腔三维分析

目的 探讨单侧完全性唇腭裂患者咽腔三维结构与正常人之间的差异.方法 应用Mimics软件对成年单侧完全性唇腭裂患者及同龄正常人的咽腔锥体束CT(cone beam computed tonoography,CBCT)数据进行分析,比较两组之间气性咽腔横径(Dtp、Dtc2、Dtc3)、前后径(Dsp、Dsc2、Dsc3)、横截面积(Ap、Ac2、Ac3)以及体积(Vt、Vp、Vp-c2、Vc2-c3)等的差异.结果 共入组 18 例单侧完全性唇腭裂术后患者及 20 例同龄正常人;单侧完全性唇腭裂患者Dsp、Ap、Vt、Vp及Vp-c2 大于正常组(P=0.000、P=0.003、P=0.021、P=0.004、P=0.000).结论 单侧完全性唇腭裂术后患者咽腔大于正常人,且差异主要是鼻咽部前后径增加所导致的鼻咽面积、体积增加.
Evaluation of pharynx in unilateral compete cleft lip and palate with CBCT
Objective To investigate the difference between the three-dimensional structure of pharyngeal cavity in patients with unilateral complete cleft lip and palate(UCLP)and normal people.Methods CBCT of patients with UCLP and normal was analyzed by Mimics.Linear(Dtp、Dtc2、Dtc3、Dsp、Dsc2、Dsc3),planar(Ap、Ac2、Ac3),and volumetric(Vt、Vp、Vp-c2、Vc2-c3)were compared between UCLP and normal.Statistical analyses were performed by using independent-samples t test,with the threshold of significance set at P=0.05.Results A total of 18 patients with unilateral complete cleft lip and palate and 20 normal controls of the same age were included.Dsp,Ap,Vt,Vp and VP-C2 in unilateral complete cleft lip and palate patients were higher than those in normal group(P=0.000,P=0.003,P=0.021,P=0.004,P=0.000).Conclusion CBCT found that pharynx was enlarged in adult patients with UCLP,and the difference was mainly due to the increase of nasopharyngeal area and volume caused by the increase of anterior and posterior diameter of nasopharynx.
