The value of neutrophil-plateletcrit ratio,fecal calprotectin and serological indicators in judging the activity of Crohn's disease
Objective:To investigate the value of neutrophil-plateletcrit ratio(NPR),fecal calprotectin(FC),D-dimer(D-D),and neutrophil-lymphocyte ratio(NLR)in judging the activity of patients with Crohn's disease(CD).Methods:We retrospectively analyzed 63 patients with Crohn's disease admitted to the Second Affiliated Hospital of Guangzhou University of Chinese Medicine from April 2019 to April 2023.The patients were grouped according to Montreal classification and Crohn disease activity index(CDAI),and the differences in FC,D-D,NLR,and NPR levels between the groups were compared.The value of each indicator in judging activity and CD's moderate to severe activity was evaluated by plotting the receiver operating characteristic curve(ROC)and calcu-lating the area under the ROC curves(AUCs).Results:The differences in the levels of FC,D-D,NLR,and NPR among CD patients in remission,mildly active,and moderately and severely active stages were statistically signifi-cant(P<0.05).There was a considerable difference in FC levels between L1 and L3 in CD patients(P<0.05),while no significant difference in levels of D-D,NLR,and NPR was found between disease locations and disease behaviors(all P>0.05).FC,D-D,NLR,and NPR levels were positively correlated with CD activity(all P<0.05).FC had the best utility in determining patients with active CD,with an AUC of 0.765(95%CI 0.644-0.887),a sensitivity of 60%,and a specificity of 91.4%.The AUCs of FC,D-D,NLR,and NPR forjudging CD's moderate to severe activity were>0.7(all P<0.05).Among them,NPR had the best utility with an AUC of 0.795(95%CI 0.682-0.908),a sensitivity of 70%,and a specificity of 84.8%.Conclusion:FC is effective in determining the degree of CD disease activity.The utility of NPR in determining patients with moderately to se-verely active CD is superior to that of FC and has the potential for disease assessment and monitoring in CD.
