中医"治未病"思想指导慢性胃炎"炎-癌"转化防治的思考与实践

摘要
中医"治未病"思想对于胃"炎-癌"转化的防治具有重要的指导意义,应将"既病防变"思想贯穿胃"炎-癌"转化全过程,时刻将"防变"作为治疗的目标,形成胃"炎-癌"转化全链条防治策略:早识恶性进展之先兆,防微杜渐,欲病救萌;在把握动态演变规律的前提下,抓住最佳时机,改变"重治疗、轻预防"的诊疗模式,化"被动"为"主动",积极主动进行干预;同时摄生护胃,调养脾胃生理功能.同时,基于"既病防变"思想防治胃"炎-癌"转化还存在着难点问题,如缺乏连续、动态、全过程的评价体系和对胃"炎-癌"转化防治的全程管理.可通过建立连续、动态的整合研究体系,以讲清楚、说明白中医药防治胃"炎-癌"转化的作用机制,建立胃"炎-癌"转化全病程专病一体化智能管理平台,实现对患者的全病程管理并获取更多循证证据.
Abstract
The idea of"preventive treatment of disease"in Traditional Chinese Medicine(TCM)has important guiding significance for the prevention and treatment of"inflammation-cancer"transformation.The idea of"pre-venting disease from changing"should be run through the whole process of"inflammation-cancer"transformation,and"preventing disease from changing"should be taken as the goal of treatment at all times to form the whole chain prevention and treatment strategy of"inflammation-cancer"transformation:early recognition of the omen of malignant progress,prevention of micro-progression,and rescue of disease;on the premise of grasping the law of dynamic evolution,we should seize the best opportunity to change the diagnosis and treatment mode of"heavy treatment,light prevention",turn"passive"into"active",and actively intervene;at the same time,it protects the stomach and regulates the physiological function of the spleen and stomach.At the same time,there are still difficult problems in the prevention and treatment of"inflammation-cancer"transformation based on the idea of"preventing the transformation of both diseases",such as the lack of continuous,dynamic and whole-process eval-uation system and the whole-process management of the prevention and treatment of"inflammation-cancer"trans-formation.Through the establishment of a continuous and dynamic integrated research system,we can clarify and explain the mechanism of TCM in the prevention and treatment of"inflammation-cancer"transformation,and es-tablish an integrated intelligent management platform for the whole course of"inflammation-cancer"transforma-tion,so as to realize the whole course management of patients and obtain more evidence-based evidence.
