目的:观察穴位埋线对功能性排便障碍(functional defecation disorder,FDD)大鼠模型的干预作用,探讨其治疗FDD的作用机制。方法:将24只雄性SD大鼠随机分为空白组、FDD模型组、穴位埋线组、假埋线组,每组6只。空白组予正常饮食饲养,其余3组采用低纤维饮食饲料联合0。1%亚甲蓝肛周注射法制备FDD模型。造模成功后穴位埋线组予穴位埋线治疗,假埋线组予穴位针刺治疗。观察各组大鼠的一般情况、粪便情况、排便模式,苏木精-伊红染色观察大鼠直肠组织病理形态改变;ELISA法测定各组大鼠血清中5-HT、IL-6、TNF-α、P物质、Ach浓度水平。免疫组化法检测大鼠直肠内降钙素基因相关肽(calcitonin gene related peptide,CGRP)蛋白的表达;实时荧光定量PCR法检测大鼠直肠组织中CGRP mRNA的表达。结果:穴位埋线组FDD大鼠粪便变软、增粗,粪便Bristol评价Ⅱ~Ⅲ型,粪便重量增加,球囊排出时间较FDD模型组增长,直肠压力较FDD模型组升高,直肠内杯状细胞减少,少量的淋巴细胞浸润在固有层、黏膜下层,少量红细胞外逸和血管扩张,5-HT、P物质、Ach水平较FDD模型组升高,IL-6、TNF-α水平较FDD模型组降低。穴位埋线组大鼠直肠中CGRP蛋白、mRNA的表达均较FDD模型组明显降低(P<0。05)。结论:穴位埋线可以有效改善FDD模型大鼠粪便性状及排便模式,其作用机制可能与调控血清脑肠肽水平、降低炎症相关递质及直肠内CGRP的表达有关。
Effect and mechanism of acupoint catgut embedding on functional defecation disorder rat model
Objective:To observe the effect and mechanism of acupoint catgut embedding on functional defeca-tion disorder(FDD)rat model.Methods:Twenty-four male SD rats were randomly divided into the blank group,FDD model group,acupoint catgut embedding group and sham embedding group.The blank group was fed with normal diet,and the other three groups were fed with low fiber diet combined with 0.1%methylene blue perianal injection to prepare FDD model.After successful modeling,the acupoint catgut embedding group received acu-point embedding treatment,while the sham embedding group received acupoint acupuncture treatment.The gener-al condition,feces condition,defecation pattern of rats in each group were observed,pathological changes of rectal tissues were observed by HE staining,and the concentrations of 5-HT,IL-6,TNF-α,substance P and Ach in ser-um of rats in each group were determined by ELISA.Calcitonin gene related peptide(CGRP)protein expression was detected by immunohistochemistry and CGRP mRNA expression was detected by Real-time PCR.Results:After acupoint catgut embedding,FDD rats stool became soft,stool thickened,Bristol evaluation type Ⅱ-Ⅲ,stool quality increased,balloon discharge time increased,rectal resting pressure increased,goblet cells decreased,a small number of lymphocytes infiltrated in lamina propria and submucosa,a small number of red blood cells es-caped and blood vessels dilated,5-HT,substance P,Ach concentration increased,IL-6,TNF-α concentration de-creased compared with FDD model group.The integrated optical density of CGRP protein in rectum of acupoint catgut embedding group was lower than that of the model group(P<0.05),and the expression of CGRP mRNA in rectum of acupoint catgut embedding group was significantly lower than that of the model group(P<0.05).Conclusion:Acupoint catgut embedding can effectively improve the fecal characteristics and defecation pattern of FDD model rats,and its mechanism may be related to regulating the level of serum brain-gut peptide,decreasing the expression of inflammatory mediators and decreasing the expression of CGRP in rectum.
