基于转录组学分析降尿酸方对痰浊瘀阻型尿酸性肾病患者NLRP3/IL-1β信号通路的影响

目的:探讨降尿酸方对痰浊瘀阻型尿酸性肾病患者PBMCs中NLRP3/IL-1β的影响.方法:从一项单中心随机对照试验中随机选取6 例受试者,其中试验组(降尿酸方联合非布司他治疗)和对照组(降尿酸方安慰剂联合非布司他治疗)各3 例受试者.对试验组和对照组治疗前后与3 例正常人群共15 例样本的外周血白细胞进行mRNA-seq分析;提取试验组和对照组患者治疗前后的外周血单个核细胞,Western blot检测PBMCs中NLRP3、IL-1β蛋白水平差异,RT-qPCR检测PBMCs中NLRP3、Caspase-1、IL-18 基因表达水平.结果:KEGG分析降尿酸方治疗后涉及到的通路有NOD样受体信号通路、NF-κB信号通路、Toll样受体信号通路等.试验组干预 12 周后PBMCs中NLRP3、IL-1β蛋白水平降低,NLRP3、Caspase-1、IL-18 基因水平明显低于对照组(P<0.05).结论:降尿酸方可以抑制痰浊瘀阻型尿酸性肾病患者NLRP3/IL-1β信号通路,减轻对肾脏的炎性损伤.
Exploring the Effect of Jiangniaosuan Formula on NLRP3/IL-1β Signaling Pathway in Hyperuricemic Nephropathy Patients with Obstruction of Phlegm Turbidity and Blood Stasis Syndrome Using mRNA-seq
Objective:To investigate the effect of Jiangniaosuan formula on NLRP3/IL-1β in PBMCs in patients with Hy-peruricemic nephropathy and with obstruction of phlegm turbidity and blood stasis.Methods:Six subjects were randomly selected from a single-center randomized controlled trial,including 3 subjects in the treatment group(Jiangniaosuan formula plus febuxostat treat-ment)and 3 subjects in the control group(Jiangniaosuan formula placebo plus febuxostat treatment).mRNA-seq analysis was per-formed on peripheral blood leukocytes from 15 samples,including 3 subjects in the normal people,3 subjects in the treatment group before and after treatment,and 3 subjects in the control group.Peripheral blood mononuclear cells were extracted from patients in the experimental group and control group before and after treatment,Western blot detected the difference in NLRP3 and IL-1β protein levels in PBMCs,and RT-qPCR detected the expression levels of NLRP3,Caspase-1 and IL-18 genes in PBMCs.Results:KEGG analysis demonstrated several signaling pathways were involved in the Jiangniaosuan formula treatment,including NOD-like receptor signaling pathway,NF-κB signaling pathway,and Toll-like receptor signaling pathway.After 12 weeks of intervention in the treatment group,the levels of NLRP3 and IL-1β proteins in PBMCs decreased,and the levels of NLRP3,Caspase-1 and IL-18 genes were significantly lower than those in the control group(P<0.05).Conclusion:Jiangniaosuan formula can inhibit NLRP3/IL-1β signaling pathway in patients with Hyperuricemic nephropathy and with obstruction of phlegm turbidity and blood sta-sis and reduce inflammatory damage to the kidneys.
