腹膜透析患者获得性肾囊肿的影响因素分析

目的:分析腹膜透析患者获得性肾囊肿的临床特点和影响因素.方法:选取2021 年02 月—2022 年02 月期间江南大学附属医院维持腹膜透析6 个月以上的住院患者共110 例,分为ACKD组和非ACKD组,比较两组患者临床特点,分析腹透患者获得性肾囊肿的影响因素.结果:110 例患者中有ACKD患者41 例(37.3%)和非ACKD患者69 例(62.7%).ACKD患者年龄(58.95±13.64)大于非ACKD患者(53.12±11.47),透析时间36(14.82～60)月大于非ACKD患者24(8.5～32.85)月,PTH水平581(287.5～1200)大于非ACKD患者411(214.5～576.5),差异均具有统计学意义(P<0.05).但两组患者在性别、BMI、高血压、糖尿病、原发病、血红蛋白、肌酐、尿素氮、尿酸、白蛋白、血脂、铁蛋白、肿瘤指标(CYFRA、CA199)、钙、磷水平方面差异均无统计学意义(P>0.05),在Kt/V、PET也差异均无统计学意义(P>0.05).二元Logistic回归分析提示年龄、透析时间、PTH是腹透患者获得性肾囊肿的影响因素,年龄越大(OR =1.076,95%CI =1.03～1.125,P =0.001),透析时间(月)越长(OR =1.024,95%CI =1.003～1.045,P =0.024),PTH水平越高(OR =1.002,95%CI =1.00～1.003,P = 0.008)越易发生获得性肾囊肿.结论:年龄、透析时间对腹透患者获得性肾囊肿的发生存在影响,年龄越大,透析时间越长,获得性肾囊肿发生的概率就较高,且较低的PTH水平可以减少获得性肾囊肿的发生.
Risk Factors of Acquired Cystic Kidney Disease in Peritoneal Dialysis Patients
Objective:To analyze the clinical characteristics and possible factors of acquired cystic kidney disease(ACKD)in peritoneal dialysis patients.Methods:A total of 110 patients undergoing peritoneal dialysis for at least six months in the Affiliated Hospital of Jiangnan University from February 2021 to February 2022 were enrolled and divided into ACKD group and without ACKD group.The clinical characteristics of acquired cystic kidney disease in peritoneal dialysis patients were analyzed and the possible fac-tors were also analyzed.Results:There were41(37.3%)patients with ACKD and69(62.7%)patients without ACKD.The mean ageinthepatientswith ACKD(58.95±13.64)was greater than in those without ACKD(53.12±11.47).The dialysis time in ACKD patients(36(14.82～60)months)was longer than those without ACKD(24(8.5～32.85)months).PTH in ACKD pa-tients(581(287.5-1200))was also higher than that without ACKD(411(214.5～576.5)).The differences were all statistically significant(P<0.05).But there was no significant difference in gender,BMI,primary affection,history of hypertension and diabe-tes.The level of hemoglobin,creatinine,urea nitrogen,uric acid,albumin,lipids,ferritin,tumor markers(CYFRA,CA199),cal-cium,phosphorum,Kt/V and PET(Peritoneal Equilibration Test)between two groups were also different(P>0.05).Binary logistic regression analysis showed that age,the duration of dialysis,and the level of PTH were risk factors of acquired cystic kidney disease in peritoneal dialysis patients.Patients with older age(OR =1.076,95%CI =1.03～1.125,P = 0.001),longer dialysis time(OR =1.024,95%CI =1.003～1.045,P =0.024),and higher level of PTH(OR =1.002,95%CI =1.00～1.003,P =0.008)are more likely to develop acquired renal cysts.Conclusion:Age and dialysis time have influence on the occurrence of acquired renal cysts in peritoneal dialysis patients.The older age and the longer dialysis time can increase the incidence of acquired renal cysts.The lower PTH level can reduce the occurrence of acquired renal cysts.
