慢性肾脏病患者血清IL-27、VASH-1水平与疾病进展的关系及其诊断价值

目的:探讨血清白介素-27(IL-27)、血管抑制蛋白-1(VASH-1)水平与慢性肾脏病患者疾病进展的关系,并分析其诊断价值.方法:选取 2021 年 6 月—2022 年 10 月在我院诊治的 280 例慢性肾脏病患者为观察组,及同期285 例健康体检志愿者为对照组.根据肾脏疾病标准将患者分为 1 期(66 例)、2 期(74 例)、3 期(52 例)、4 期(50 例)、5 期(38 例)慢性肾脏病组.比较慢性肾脏病患者血清IL-27、VASH-1 水平,并分析血清IL-27、VASH-1 水平及二者与血肌酐(Scr)、肾小球滤过率(e-GFR)以及慢性肾脏病分期的相关性;对影响慢性肾脏病的因素进行Logistic回归分析;受试者工作特征(ROC)曲线分析血清IL-27、VASH-1 水平对慢性肾脏病的诊断价值.结果:与对照组相比,1 期、2 期、3 期、4 期、5 期慢性肾脏病组患者血清IL-27、VASH-1、Scr水平均依次显著升高(P<0.05),e-GFR依次明显降低(P<0.05).慢性肾脏病患者血清IL-27、VASH-1 与Scr水平均呈正相关(r=0.672,0.684,P<0.05),与e-GFR均呈负相关(r=-0.691,-0.679,P<0.05),与慢性肾脏病分期均呈正相关(r=0.668,0.659,P<0.05);IL-27 与 VASH-1 水平呈正相关(r=0.696,P<0.05).尿素氮(BUN)、胱抑素C(Cys C)、IL-27、VASH-1 是影响慢性肾脏病的危险因素(P<0.05).血清IL-27、VASH-1 联合诊断慢性肾脏病的曲线下面积(AUC)为 0.943,均优于其各自单独诊断(Z二者联合-IL-27=1.940,P=0.024;Z二者联合-VASH-1=2.633,P=0.010).血清IL-27、VASH-1 联合诊断慢性肾脏病1 期的AUC为0.914,二者联合均优于其各自单独诊断(Z二者联合-IL-27=1.717,P=0.043;Z二者联合-VASH-1=2.557,P=0.005).结论:慢性肾脏病患者血清IL-27、VASH-1 水平均升高,与慢性肾脏病进展有关,对慢性肾脏病具有一定的诊断价值.
The Relationship Between Serum IL-27,VASH-1 Levels and Disease Progression in Patients with Chronic Kidney Disease and Their Diagnostic Value
Objective:To investigate the relationship between the levels of serum interleukin-27(IL-27),vasohibin-1(VASH-1)and the progression of chronic kidney disease,and analyze their diagnostic value.Methods:A total of 280 patients with chronic kidney disease diagnosed and treated in our hospital from June 2021 to October 2022 were selected as the observation group,and 285 healthy volunteers in the same period were regarded as the control group.According to the standard of kidney disease,the pa-tients were divided into 66 cases in stage 1 group,74 cases in stage 2 group,52 cases in stage 3 group,50 cases in stage 4 group,and 38 cases in stage 5 group.To compare serum IL-27 and VASH-1 levels in patients with chronic kidney disease(CKD)and to analyze the correlation between serum IL-27 and VASH-1 levels and both with blood creatinine(Scr),glomerular filtration rate(e-GFR),and the stage of CKD;Logistic regression analysis was conducted on the factors affecting chronic kidney disease;the di-agnostic value of serum IL-27 and VASH-1 levels in chronic kidney disease was analyzed by ROC curve.Results:Compared with the control group,the levels of IL-27,VASH-1 and Scr in serum of patients with chronic kidney disease in stage 1 group,stage 2 group,stage 3 group,stage 4 group and 5 stage group were obviously higher(P<0.05),and e-GFR was obviously lower(P<0.05).Serum IL-27 and VASH-1 were positively correlated with Scr levels in patients with chronic kidney disease(r=0.672,0.684,P<0.05),and negatively correlated with e-GFR(r=-0.691,-0.679,P<0.05),positively correlated with the stages of chronic kidney disease(r=0.668,0.659,P<0.05),IL-27 was positively correlated with VASH-1(r=0.696,P<0.05).BUN,Cys C,IL-27 and VASH-1 were risk factors for chronic kidney disease(P<0.05).The area under the curve(AUC)of serum IL-27 combined with VASH-1 in the diagnosis of chronic kidney disease was 0.943,which was better than their respective diagnosis alone(Zcombination-IL-27=1.940,P=0.024;Zcombination-VASH-1=2.633,P=0.010).The AUC of serum IL-27 combined with VASH-1 in the diagnosis of chronic kidney disease stage 1 was 0.914,which was better than their respective diagnosis alone(Zcombination-IL-27=1.717,P=0.043;Zcombination-VASH-1=2.557,P=0.005).Conclusion:The levels of serum IL-27 and VASH-1 in patients with chronic kidney disease are elevated,they are related to the progress of chronic kidney disease and have certain diag-nostic value for chronic kidney disease.
