Exploring the Medication Pattern and Mechanism of Action of Traditional Chinese Medicine in the Treatment of Severe Purpura Nephritis Based on 384 Cases
Objective:To study the medication pattern and mechanism of action of traditional Chinese medicine in the treat-ment of severe Henoch-Schönlein purpura nephritis ( HSPN) . Methods:Collect medical record data of patients with severe purpura nephritis who visited the First Affiliated Hospital of Henan University of Traditional Chinese Medicine from January 1,2020 to August 31,2023,and summarize the traditional Chinese medicine prescriptions with the highest 24-hour urinary protein quantification,and conduct statistical analysis on medication frequency and taste. Obtaining the effective ingredients and targets of drugs through TCMSP databases. Obtain HSPN targets through disease databases,and intersect drug targets with disease targets through Venny. Perform PPI analysis using String database. Using the Metascape database for KEGG and GO enrichment analysis. Finally,the results were veri-fied using molecular docking technology. Results:A total of 384 prescriptions were collected,after undergoing a combination of tradi-tional Chinese and Western medicine treatment for 2 weeks,urinary protein,occult blood,and 24-hour urine protein quantification all showed a significant decrease compared to before (P<0.01). The traditional Chinese medicine prescriptions include 213 tradi-tional Chinese medicines with warm or cold nature and sweet and light taste. They belong to the liver,spleen,kidney,and lung me-ridians,and the use of Radix astragali-Tuckahoe-Cornel ranks first. The active ingredients of high-frequency medicine Radix as-tragali-Tuckahoe-Cornel are mainly quercetin,kaempferol,hederagenin etc. After screening,21 core targets were obtained from HSPN,with the main targets being TNF,IL-6,IL-β1,AKT1,and MMP9. The main pathways of action are AGE-RAGE signa-ling pathway,NF-κB signaling pathway,etc. The molecular docking results proved that the docking performance of the active ingre-dient and the core target was good. Conclusion:The main drugs used in the treatment of severe purpura nephritis with traditional Chi-nese medicine are mostly to strengthen the spleen and kidney,cool blood and stop bleeding,and promote blood circulation and remove stasis. It acts on TNF,IL-6,IL-β1,AKT1,MMP9 through components such as quercetin,kaempferol,and hederagenin,and participates in the inhibition of inflammatory response and regulation of cell apoptosis to treat severe purpura nephritis.
