Comparative analysis of quantitative parameters among different coronary arteries by IVIM imaging in healthy miniature pig hearts

目的:探讨健康小型猪心脏IVIM成像中不同冠状动脉间各定量参数的标准值.方法:采用3.0 T MRI FIESTA序列对10只健康小型猪心脏行标准二、三、四腔心及左心室短轴电影成像,在短轴层面上再对心底、心中及心尖部分别行IVIM成像.由2位影像医师运用双盲法评估图像质量、划分等级,并分别测量不同冠状动脉供血区域的相关参数(D、D*、f).结果:2位医师对图像质量等级评估的一致性较好(K=0.84).右冠状动脉(RCA)与左旋支(LCX)、左前降支(LAD)与左回旋支(LCX)间供血区域心肌的D*值差异均有统计学意义(均P<0.05);RCA与LCX间f值差异有统计学意义(P<0.05);不同冠状动脉供血区域间的D值差异无统计学意义(P>0.05).D*值、D值和f值的AUC分别为0.625、0.488和0.342.结论:不同冠状动脉供血区域各定量参数间有差异,各参数中D*值对正常心肌的诊断效能最佳,可获得相应心肌节段各定量参数的标准值,为相关研究提供参考依据.
Objective:To investigate the standard values of each quantitative parameter among different coronary arteries for IVIM imaging of the miniature pig heart.Methods:Standard two-chamber,three-chamber,four-chamber heart and short-axis cine imaging of the left ventricle were performed using 3.0 T MRI FIESTA sequence in 10 healthy miniature pig.At the level of the short axis,IVIM was performed on the bottom,heart and apex.The image quality was assessed by two radiologists using a double-blind method to grade the image quality and to measure the relevant parameters(D,D*,f values)in different coronary blood supply areas separately.Results:The consistency of the image quality assessment was good(K=0.84).The differences in the D* values between RCA and LCX and between LAD and LCX were statistically significant(both P<0.05).The difference in f value between RCA and LCX was statistically significant(P<0.05).The difference in D values between different coronary supply regions was not statistically significant(P>0.05).The AUCs for D*,D and f vlaues were 0.625,0.488 and 0.342,respectively.Conclusions:There are differences in the quantitative parameters among the three major branches of the coronary artery supplying the myocardium according to IVIM imaging of the miniature pig heart,and the D* value has the best diagnostic efficacy for normal myocardium.The normal parameters of the corresponding myocardial segments are also obtained,which can provide a basis for relevant studies.
