Objective:To analyze and compare the imaging characteristics of granuloma mastitis(GM)and breast cancer with non-mass-like enhancement(NME)on MRI,and improve the ability of differential diagnosis.Methods:A total of 19 GM patients(21 lesions)and 33 breast cancer patients(35 lesions)were retrospectively collected,all lesions were NME showed on dynamic contrast-enhanced MRI.The MRI main signs,additional signs and ring enhancement characteristics of lesions of the two groups were evaluated.Results:Compared with breast cancer,GM patients were younger and had a larger range of lesions,the main manifestations of NME internal enhancement were multi-regional(38.1%)and clustered-ring enhancement(66.7%),and the enhanced ring of GM lesion was even larger and the ratio of the wall to the ring was even smaller,and the enhanced ring had no significant change in the early or late stages of the enhancement scan,and the skin thickening around the areola,fistula or sinus formation were also more common(all P<0.05).Compared with GM lesions,the internal enhancement of NME in breast cancer was mainly segmental(45.7%)and clustered-ring enhancement(57.1%),the cluster ring enhancement was more common in the late stage(70.0%),and the structural deformation was more likely to occur around the lesions(all P<0.05).Conclusions:It is difficult to differentiate GM and breast cancer with NME on MRI,but it is important to accurately grasp the key MRI signs including NME enhancement features,clustered-ring enhancement features and so on.
