骨复生胶囊联合髓芯减压自体髂骨植骨治疗早期股骨头坏死的临床研究

目的:观察骨复生胶囊联合髓芯减压自体髂骨植骨术治疗早期股骨头坏死的临床效果。方法:选取2014年1月至2020年6月诊断为股骨头坏死(ARCOⅡ A~ⅢA期)的住院患者70例(83髋),按照随机数字表法分为两组,每组35例,对照组采用髓芯减压自体髂骨植骨术治疗,观察组在其基础上口服骨复生胶囊6个月,观察两组在治疗12个月后的视觉模拟量表(VAS)评分、髋关节功能日本骨科协会(JOA)评分、国际骨循环学会(ARCO)分期影像学表现变化。结果:治疗12个月后两组患者VAS评分、髋关节功能JOA评分均有明显改善,差异有统计学意义(P<0。05),且观察组优于对照组,差异有统计学意义(P<0。05);两组患者ARCO分期影像学表现也均有改善,差异有统计学意义(P<0。05),且观察组改善明显优于对照组,差异有统计学意义(P<0。05)。结论:骨复生胶囊联合髓芯减压自体髂骨植骨术治疗早期股骨头坏死能显著提高髋关节功能评分,缓解髋关节疼痛,改善影像学特征,比单纯髓芯减压植骨术具有更好的临床效果。
Clinical Observation on Early Osteonecrosis of the Femoral Head Treated by Core Decompression and Autogenous Iliac Bone Grafting Combined with Gufusheng Capsule
Objective:To observe the efficiency of Gufusheng capsule combined with core decompression and autogenous ili-ac bone grafting at the femoral head-neck junction on the treatment of osteonecrosis of the femoral head(ONFH).Meth-ods:70 cases of patients(83 hips)diagnosed with femoral head necrosis(ARCO Ⅱ A-Ⅲ A stage)were selected from Janu-ary 2014 to June 2020.They were divided into two groups according to the random number table method,with 35 cases in each group.The control group was treated with only core decompression and autogenous bone grafting.The observation group received additionally treated with Gufusheng capsule based on the treatment of the control group.The pain improve-ments of the 2 groups before and after operation were assessed by the visual analogue scale(VAS).The functions of hip joint in 2 groups before and after operation were evaluated by Japanese orthopaedic association(JOA)score.And imaging methods was used to observed the morphological changes of the femoral head.Results:The post-operative VAS scores of the 2 groups decreased significantly(P<0.05)comparing with the pre-operative scores.The VAS score of the observa-tion group decreased more significantly,and the difference between the two groups was statistically significant(P<0.05).The post-operative JOA scores increased significantly(P<0.05)comparing with the pre-operative scores.The JOA scores in the observation group were slightly higher than those in the control group with statistical significance(P<0.05).The post-operative imaging performance of the femoral head in the 2 groups was significantly improved(P<0.05),and the difference between the control group and the observation group was statistically significant(P<0.05).Conclusion:The therapy of Gufusheng capsule com-bined with core decompression and autogenous iliac bone grafting at the femoral head-neck junction for patients with osteonecrosis of the femoral head can effectively improve the functions of hip joint,alleviate the clinical symptoms,improve the imaging performance of the femoral head,and has significant therapeutic efficacy.The recent clinical efficacy of Gufusheng capsule combined with core decompression and autogenous iliac bone grafting is superior to core decompression and autogenous iliac bone grafting.
