筋骨平衡理论指导下五步整膝手法治疗膝骨关节炎浅析

摘要
膝骨关节炎的发生与膝关节周围肌群肌力减弱,髌股、胫股关节不稳以及下肢力线改变等因素密切相关.筋骨平衡理论认为筋骨失衡是膝骨关节炎的重要发病机制,可将其病机进展归纳为筋失衡、筋与骨失衡、骨失衡三个阶段.手法治疗是膝骨关节炎治疗指南推荐疗法,关于膝骨关节炎的治疗手法,筋骨平衡理论强调柔筋为先,筋骨并重,以痛为腧,整体施治.本文旨在从筋骨平衡理论出发,探讨膝关节筋骨结构特点及膝骨关节炎发病机制,并浅析基于筋骨平衡理论指导下的五步整膝手法治疗膝骨关节炎思路,以期为筋骨平衡理论的发展与临床实践提供新思路.
Abstract
The occurrence of knee osteoarthritis is closely related to factors such as weakened muscle strength around the knee joint,instability of the patellofemoral and tibiofemoral joints,and changes in lower limb force lines.The theory of muscle and bone balance suggests that imbalance between muscle and bone is an important pathogenesis of knee osteoar-thritis,and its progression can be summarized into three stages:muscle imbalance,muscle and bone imbalance,and bone imbalance.Manual therapy is a recommended treatment for knee osteoarthritis according to relevant guidelines.The theory of muscle and bone balance emphasizes the importance of soft tendons and a balance between muscles and bones,with pain as the acupoint and holistic treatment.This article aims to explore the characteristics of knee joint musculoskeletal struc-ture and the pathogenesis of knee osteoarthritis based on the theory of muscle and bone balance.It also analyzes the five step knee manipulation in the treatment of knee osteoarthritis based on the theory of muscle and bone balance,in order to provide new ideas for the development and clinical practice of muscle and bone balance theory.
