宣白承气汤对痰热壅肺型慢性阻塞性肺疾病急性加重期患者肠道微生态及炎症因子的影响
Effects of Xuanbai Chengqi Decoction on Gut Microbiota and Inflammatory Cytokines in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease of Phlegm-heat Obstructing Lung Type

摘要
目的 观察宣白承气汤对慢性阻塞性肺疾病急性加重期(AECOPD)(痰热壅肺证)的临床疗效、肠道微生态、炎症指标及二氧化碳分压(PaCO2)的影响.方法 将80例AECOPD(痰热壅肺证)患者随机分为两组,对照组实施西医的常规治疗,观察组则另外加上宣白承气汤治疗,疗程均为7 d,观察并记录两组患者治疗前后中医证候积分、肠道微生物菌种丰度、C反应蛋白(CRP)、降钙素原(PCT)、中性粒细胞百分比(N％)及PaCO2的变化.结果 观察组临床疗效显著优于对照组(P＜0.05).治疗后,观察组咳嗽、喘息等中医证候积分显著低于对照组(P＜0.05),两组患者乳杆菌、拟杆菌、双歧杆菌以及肠杆菌丰度均升高(P＜0.05),且与对照组比较,观察组患者乳杆菌、拟杆菌、双歧杆菌以及肠杆菌丰度明显升高(P＜0.05).两组PaCO2、CRP、PCT、N％较治疗前均降低(P＜0.05),且低于同期对照组(P＜0.05).结论 宣白承气汤可改善AECOPD(痰热壅肺证)患者的临床症状及肠道微生态,可一定程度改善患者二氧化碳潴留,并有效降低患者CRP、PCT、N％比水平.
Abstract
Objective:To analysis the effect of Xuanbai Chengqi Decoction on the clinical efficacy,gut microbio-ta,inflammatory parameters and partial pressure of carbon dioxide(PaCO2)in acute exacerbation of chronic ob-structive pulmonary disease(AECOPD)with phlegm-heat obstructing the lung.Methods:A total of 80 patients with AECOPD with phlegm-heat obstructing lung type were randomly divided into two groups.The control group was given routine western medicine treatment.The experimental group also received Xuanbai Chengqi Decoction on the basis of routine western medicine treatment.After treatment for 7 days,the traditional Chinese medicine(TCM)symptom scores,gut microbiota abundance,C-reactive protein(CRP),procalcitonin(PCT),neutrophil per-centage(N％)and changes in PaCO2 were recorded.Results:The clinical efficacy of the experimental group was significantly better than that of the control group(P＜0.05).After treatment,the TCM symptom score of cough and wheezing in the experimental group were significantly lower than that of the control group(P＜0.05).The abun-dances of lactobacillus,bacteroides,bifidobacterium and enterobacteriaceae were increased in both groups,and the increases in the experimental group were more prominent than those in the control group(P＜0.05).The PaCO2,CRP,PCT and N％were all significantly decreased compared with before treatment(P＜0.05),and the decreases in the experimental group were more significant(P＜0.05).Conclusion:Xuanbai Chengqi Decoction can improve the clinical symptoms and gut microbiota of patients with AECOPD with phlegm-heat obstructing the lung type,de-crease carbon dioxide retention to a certain extent,and effectively reduce the CRP,PCT and N％levels.
