《赤厓医案》急危重症证候真假辨识思维评析

摘要
证候真假多见于急危重症,要求医者临证透过假象把握本质,正确决策.清代名医汪廷元所著《赤厓医案》一书中记载急危重症甚多,其脉症合参,辨治精详,可惜证候真假辨证细微之处论述稍嫌简略.本文结合书中急危重症验案进行评析,对于汪氏脉症合参叙述简略之处予以申明,脉理、舌象阐发不详之处加以补充,病程演变未曾论说之处佐以分析,意在还原其辨识急危重症证候真假之临床思维过程,共分虚实真假辨、寒热真假辨、外感内伤辨、脉厥病机辨等4个部分进行阐述,以期为当今临床以及临床实践教学提供助力.
Abstract
The true and false syndromes are more common in acute and critical illness,which requires doctors to grasp the essence and make correct decisions through the false appearance.Wang Tingyuan,a famous doctor in the Qing Dynasty,recorded many acute and critical diseases in his book Chiya Medical Records.In his book,the pulses and symptoms were cross-referenced,and the differentiation and treatment were detailed.But it is a pity that the discussion of the true and false syndrome differentiation was rather brief.Based on the analysis of the acute and critical cases in the book,this paper expounds the brief description of Wang's pulse and syndrome refer-ence,supplements the unclear explanation of pulse theory and tongue image,and analyzes the undiscussed evolu-tion of the course of disease.It is intended to restore the clinical thinking process of identifying the true and false syndromes of critical illness,which is divided into four parts:true and false identification of deficiency and ex-cess,true and false identification of cold and heat,identification of external and internal injury,and pathogenesis identification of pulse syncope,so as to provide assistance for clinical practice and clinical teaching.
