摘要
目的 观察补阳还五汤加减对冠心病心绞痛(气虚血瘀证)患者心功能、炎症因子及应激指标的影响.方法 160例冠心病心绞痛(气虚血瘀证)患者随机分为两组.对照组采用常规疗法,观察组在对照组基础上联合补阳还五汤治疗,比较两组的临床疗效、中医证候积分、心功能实验室和超声指标[氨基末端脑利钠肽前体(NT-proBNP)、心肌肌钙蛋白Ⅰ(cTnⅠ)、肌酸激酶同工酶(CK-MB)、左心室舒张末期内径(LVEDD)、左室舒张末期容积(LVEDV)、左室射血分数(LVEF)、心脏指数(CI)];炎症因子[超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、单核细胞与高密度脂蛋白胆固醇比值(MHR)]及应激指标[超氧化物歧化酶(SOD)、丙二醛(MDA)、内皮素-1(ET-1)、皮质醇(COR)、胱抑素C(Cys C)].结果 治疗后,观察组的中医证候评分明显低于对照组(P＜0.05);观察组治疗后的NT-proBNP、cTnⅠ、CK-MB、LVEDD、LVEDV、hs-CRP、TNF-α、IL-6、MHR、MDA、ET-1、COR 和 Cys C 均低于对照组(P＜0.05);观察组治疗后的 LVEF、CI、SOD均明显高于对照组(P＜0.05);观察组的总有效率明显高于对照组(P＜0.05).结论 补阳还五汤加减方可明显改善冠心病心绞痛(气虚血瘀证)患者心功能,减轻炎症及应激反应,进一步改善患者的中医证候,提高临床疗效.
Abstract
Objective:To observe the effect of Modified Buyang Huanwu Decoction on heart function,inflamma-tory factors,and stress indicators in patients with coronary heart disease angina pectoris(syndrome of blood stasis due to qi deficiency).Methods:160 patients with coronary heart disease angina pectoris(syndrome of blood sta-sis due to qi deficiency)were divided into the control group and observation group by random number table meth-od.The control group was treated with conventional treatment while the observation group was given Modified Buy-ang Huanwu Decoction on the basis of the control group.The clinical efficacy,TCM syndrome scores,heart func-tion laboratory and ultrasound indicators[N-terminal pro brain natriuretic peptide(NT-proBNP),cardiac Tropo-nin Ⅰ(cTnⅠ),creatine kinase isozyme(CK-MB),left ventricular end diastolic diameter(LVEDD),left ventricular end diastolic volume(LVEDV),left ventricular ejection fraction(LVEF),cardiac index(CI)];inflammatory indexes[high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),ratio of monocyte to high-density lipoprotein cholesterol(MHR)];stress indicators[superoxide dismutase(SOD),malondi-aldehyde(MDA),endothelin-1(ET-1),cortisol(COR),cystatin C(Cys C)]were compared between the two groups.Results:The TCM syndrome scores of the observation group were significantly lower than those of the con-trol group(P＜0.05);the NT-proBNP,cTnⅠ,CK-MB,LVEDD,LVEDV,hs-CRP,TNF-α,IL-6,MHR,MDA,ET-1,COR and Cys C of the observation group were lower than those of the control group(P＜0.05);after treatment,the LVEF,CI,and SOD of the observation group showed higher levels than the control group(P＜0.05);the total effec-tive rate of the observation group was significantly higher than the control group(P＜0.05).Conclusion:Modified-Buyang Huanwu Decoction can significantly improve the heart function with angina pectoris of coronary artery dis-ease(syndrome of blood stasis due to qi deficiency),reduce inflammation and stress reaction.further improve the TCM syndrome,and improve clinical efficacy.
