Exploration of the"Tong-Bu Sequential Method"for Staged Treatment of Ulcerative Colitis in Traditional Chinese Medicine
Ulcerative colitis falls under the categories of"intestinal purgation""dysentery"and"intestinal car-buncle"in Traditional Chinese Medicine(TCM).Its core pathogenesis revolves around spleen deficiency and abun-dant dampness,where deficiency of the spleen is fundamental,and turbid toxicity is the superficial manifestation.Due to complex etiology and pathogenesis,it often presents with alternating episodes of exacerbation and remis-sion,making it difficult to resolve.Based on the physiological characteristic of"Fu functions best when unobstruct-ed",we propose the"Tong-Bu Sequential Method"for staged treatment of ulcerative colitis.The"Tong"aspect fo-cuses on eliminating real evils such as dampness,heat,toxins,and stasis,while the"Bu"component aims to rein-force vital energy to expel evils,restoring the normal circulation of qi and the true essence of the organs.In clini-cal practice,we combine western medical staging with TCM pattern identification,prioritizing the identification of stage,then clarifying the balance between deficiency and excess,before determining the emphasis on Tong or Bu.During the exacerbation phase,the primary pathogenesis is the predominance of evil and abundance of spleen dampness,thus employing both Tong and Bu.In the remission phase,deficiency of the spleen and kidney with lin-gering evil takes precedence,hence focusing on Bu with concurrent Tong.The Tong-Bu method is applied through-out,ensuring that Tong does not harm the vital energy,and Bu does not cause stagnation.This leads to a diagnos-tic and therapeutic concept of"sequential Tong-Bu,treating acute and chronic stages separately",achieving favor-able clinical outcomes.
