摘要
目的 观察毛桃二陈饮治疗慢性阻塞性肺疾病急性加重期(AECOPD)的临床疗效及对炎症指标、血气分析指标的影响.方法 将60例AECOPD患者按照随机数字表法分为常规组与毛桃组各30例.常规组视病情给予吸氧、舒张支气管、抗炎等西医常规综合治疗.毛桃组在西医常规综合治疗基础上,给予中药内服毛桃二陈饮治疗.两组均治疗2周,2周后评价临床疗效、中医症状积分、炎症指标、血气分析指标的变化.结果 治疗后毛桃组总有效率为90.00％,高于常规组的76.67％(P＜0.05).治疗后,两组咳嗽、咯痰、喘息、胸闷、乏力积分及总积分均较治疗前明显降低(P＜0.01),且毛桃组咳嗽、咯痰、胸闷、乏力积分较常规组明显降低(P＜0.05).两组治疗后血清C反应蛋白(CRP)、降钙素原(PCT)、白细胞介素-6(IL-6)均较治疗前明显降低(P＜0.01),且毛桃组CRP、PCT、IL-6水平均较常规组显著下降(P＜0.05).治疗后,两组血气分析指标氧分压(PaO2)、二氧化碳分压(PaCO2)均较治疗前明显改善,且毛桃组较常规组改善更明显(P＜0.05).结论 毛桃二陈饮联合西医常规综合疗法能有效改善AECOPD患者临床症状,降低炎症指标,改善血气分析指标,提高临床疗效.
Abstract
Objective:To observe the clinical efficacy of Maotao Erchen Decoction in treating patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and its effects on inflammatory markers and arterial blood gas analysis.Methods:60 AECOPD patients were randomly divided into a conventional group and a Maotao group,each consisting of 30 cases.The conventional group received standard western medical treat-ments including oxygen therapy,bronchodilators,and anti-inflammatory drugs according to their condition.The Maotao group received the same standard western medical treatments combined with oral administration of Maotao Erchen Decoction.Both groups were treated for two weeks,after which the clinical efficacy,traditional Chinese med-icine symptom scores,inflammatory markers,and arterial blood gas analysis results were evaluated.Results:After treatment,the overall effective rate was 90.00％in the Maotao group,which was significantly higher than 76.67％in the conventional group(P＜0.05).After treatment,both groups showed significant reductions in cough,sputum production,dyspnea,chest tightness,fatigue scores,and total scores compared with before treatment levels(P＜0.01),with the Maotao group demonstrating significantly lower scores for cough,sputum production,chest tight-ness,and fatigue compared with the conventional group(P＜0.05).Serum levels of C-reactive protein(CRP),pro-calcitonin(PCT),and interleukin-6(IL-6)decreased significantly after treatment in both groups(P＜0.01),with the Maotao group showing more significant reductions in CRP,PCT,and IL-6 levels compared with the convention-al group(P＜0.05).After treatment,both groups showed significant improvements in arterial blood gas analysis pa-rameters,including partial pressure of oxygen(PaO2)and carbon dioxide(PaCO2),with the Maotao group showing more significant improvements compared with the conventional group(P＜0.05).Conclusion:The combination of Maotao Erchen Decoction with standard western medical therapy effectively improves clinical symptoms in AECO-PD patients,reduces inflammation markers,improves arterial blood gas analysis parameters,and enhances clinical efficacy.
