目的 运用证据图谱的方法系统全面地收集、分析和评价中医药治疗急性痛风性关节炎的相关证据,发现该领域证据的现状与不足,为研究的进一步开展和临床实践提供支持.方法 检索中国知网(CNKI)、万方数据知识服务平台、中文科技期刊数据库(VIP)、中国生物医学文献服务系统(SinoMed)、Web of Science、PubMed、Embase、Cochrane Library截至2024年6月收录的中医药治疗急性痛风性关节炎的系统评价/Meta分析,描述纳入研究的基本特征和研究问题的特征,使用气泡图呈现纳入研究的结论和方法学质量.结果 共纳入23篇系统评价/Meta分析,主要包括中药内服、中药外敷、针灸和中西医结合4种干预类型,结局指标以总有效率、不良反应、血尿酸、血沉、C反应蛋白和疼痛视觉模拟评分为主.AMSTAR 2量表评价结果中10篇文献为极低级,12篇为低级,1篇为中级.结论 中医药治疗急性痛风性关节炎的相关研究方法多样,但对中医特色结局指标使用缺乏,证据整体质量不高,需进一步提高相关研究的方法学质量,完善证据系统,促进临床实践发挥中医优势.
Evidence Map of Traditional Chinese Medicine for Acute Gouty Arthritis
Objective:To systematically collect,analyze and evaluate the evidence related to the treatment of acute gouty arthritis with traditional Chinese medicine(TCM)through an evidence map,and to identify the current status and shortcomings of evidence in this field,providing support for further research and clinical practice.Meth-ods:CNKI,Wanfang Data,VIP,SinoMed,Web of Science,PubMed,Embase and Cochrane Library were searched for systematic reviews/meta-analyses on TCM treatments for acute gouty arthritis published up to June 2024.The basic characteristics of the research and the characteristics of the research questions were detailed described,the conclusions and methodological quality were presented by bubble diagram.Results:A total of 23 systematic re-views/meta-analyses were included,mainly covering four types of interventions:oral TCM,topical TCM,acupunc-ture,and integrated TCM and western medicine treatments.The outcome measures primarily included overall effec-tiveness rate,adverse reactions,serum uric acid levels,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),and visual analog scale(VAS)pain scores.According to the AMSTAR 2 assessment,10 articles were rat-ed as very low quality,12 as low quality,and 1 as moderate quality.Conclusion:Studies on TCM treatments for acute gouty arthritis employ diverse methodologies but lack the use of TCM-specific outcome indicators.The over-all quality of evidence is not high,and there is a need to further improve the methodological quality of related re-search,perfect the evidence system,and promote the advantages of TCM in clinical practice.
