大柴胡汤对CCl4诱导大鼠急性肝损伤炎症反应的调控作用

目的 观察大柴胡汤对急性肝损伤的保护作用.方法 36只雄性SD大鼠按随机数字表法分为正常(NOM)组、模型(CCl4)组、熊去氧胆酸(UDCA)组及大柴胡汤低、中、高剂量(DCH-L、M、H)组,每组6只.采用ELISA检测肝功能指标;全自动生化仪检查各组大鼠血清炎症因子及氧化应激指标;HE、天狼星红染色观察各组大鼠病理变化;实时荧光PCR检测MIP-2、MIP-1β mRNA表达;WB检测大鼠巨噬细胞言行蛋白-2(MIP-2)蛋白表达.结果 与NOM组比较,CCl4组碱性磷酸酶(ALP)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、γ-谷氨酰转肽酶(γ-GT)、直接胆红素(DBIL)、硫代巴比妥酸(TBA)及总胆红素(T-BIL)水平显著升高(P<0.01),白细胞介素-6(IL-6)、转化生长因子-β(TGF-β)及肿瘤坏死因子-α(TNF-α)水平升高(P<0.01),超氧化物歧化酶(SOD)、谷胱氨肽(GSH)水平显著降低,MDA水平显著升高(P<0.01);MIP-2、MIP-1β mRNA水平显著增高(P<0.01),MIP-2蛋白阳性表达显著增强(P<0.01).HE染色结果肝细胞体积增大,伴有不同程度炎症细胞浸润、坏死,可见气球样变或纤维样变;天狼星红染色结果示肝组织胶原纤维明显增生,胶原染色面积增大.与CCl4组比较,UDCA及DCH-L、M、H组均能逆转这些指标及病理改变,且高剂量组效果更优.结论 大柴胡汤可以改善CCl4大鼠肝功能指标及病理改变,降低炎症因子水平,并抑制MIP-1β、MIP-2趋化因子的表达.其可能的机制为通过抑制炎症反应从而对CCL4大鼠急性肝损伤发挥保护作用.
Regulation Effect of Dachaihu Decoction on Inflammatory Response in CCl4-Induced Acute Liver Injury in Rats
Objective:To investigate the protective effect of Dachaihu Decoction on acute liver injury.Meth-ods:Thirty-six male SD rats were randomly divided into normal(NOM)group,model(CCl4)group,ursodeoxycho-lic acid(UDCA)group,and low(DCH-L),medium(DCH-M),high(DCH-H)dose groups of Dachaihu Decoction,with six rats in each group.Liver function indicators were detected by ELISA;serum inflammatory factors and oxi-dative stress indicators of all groups were examined using an automatic biochemical analyzer;pathological changes in all groups were observed by HE and Sirius Red staining;MIP-2 and MIP-1β mRNA expressions were detected by real-time fluorescence PCR;macrophage inflammatory protein-2(MIP-2)protein expression was detected by Western blotting(WB).Results:Compared with the NOM group,the levels of alkaline phosphatase(ALP),alanine transaminase(ALT),aspartate transaminase(AST),γ-glutamyl transpeptidase(γ-GT),direct bilirubin(DBIL),thio-barbituric acid(TBA),and total bilirubin(T-BIL)in the CCI4 group were significantly increased(P<0.01),while interleukin-6(IL-6),transforming growth factor-β(TGF-β),and tumor necrosis factor-α(TNF-α)levels were al-so elevated(P<0.01).Superoxide dismutase(SOD)and glutathione(GSH)levels were significantly decreased,and MDA levels were significantly increased(P<0.01).The levels of MIP-2 and MIP-1β mRNA were significantly increased(P<0.01),and MIP-2 protein expression was significantly enhanced(P<0.01).Histological examination showed hepatocyte enlargement accompanied by varying degrees of inflammatory cell infiltration and necrosis,bal-loon-like or fibrous changes were visible;Sirius Red staining showed obvious collagen fiber proliferation and in-creased collagen staining area.Compared with the CCl4 group,UDCA,DCH-L,DCH-M,and DCH-H groups could reverse these indicators and pathological changes,with the high-dose group showing better effects.Conclusion:Dachaihu Decoction can improve liver function indicators and pathological changes in CCL4-induced rats,reduce the levels of inflammatory factors,and inhibit the expression of chemokines MIP-1β and MIP-2.Its potential mechanism is through inhibition of the inflammatory response,thereby exerting a protective effect against acute liv-er injury in CCl4-induced rats.
