解毒泻肺合剂治疗老年重症肺炎(痰热壅肺证)临床观察

摘要
目的 观察解毒泻肺合剂治疗老年重症肺炎(痰热壅肺证)患者的疗效并探寻其作用机制.方法 将60例患者采用随机数字表法分为对照组与观察组各30例.对照组给予西医常规治疗,观察组在西医常规治疗的基础上予解毒泻肺合剂鼻饲,治疗周期14 d.比较两组治疗效果.结果 治疗后观察组中医整体有效率为80.00％,高于对照组的63.33％(P＜0.05);观察组治疗后C反应蛋白(CRP)、白细胞计数(WBC)、白细胞介素-6(IL-6)下降更明显(P＜0.05),急性生理与慢性健康评分(APACHE Ⅱ评分)、临床肺部感染评分(CPIS评分)改善更显著(P＜0.05).结论 解毒泻肺合剂可减轻炎症反应,改善临床症状,治疗老年重症肺炎(痰热壅肺证)患者疗效显著.
Abstract
Objective:To observe the efficacy of the Jiedu Xiefei mixture in treating elderly patients with severe pneumonia(syndrome of phlegm-heat obstructing lung)and to explore its mechanism of action.Methods:60 pa-tients were randomly divided into a control group and an observation group of 30 patients each using the random number table method.The control group received routine western medical treatment,while the observation group re-ceived Jiedu Xiefei mixture via nasogastric tube in addition to western medical treatment.The treatment duration was 14 days.The therapeutic effects of both groups were compared.Results:After treatment,the overall TCM ef-fective rate in the observation group was 80.00％,which was higher than 63.33％in the control group(P＜0.05).Additionally,the observation group showed a more significant decrease in C-reactive protein(CRP),white blood cell count(WBC),and interleukin-6(IL-6)levels(P＜0.05),and more remarkable improvements in the Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ)score and Clinical Pulmonary Infection Score(CPIS)(P＜0.05)compared with the control group.Conclusion:Jiedu Xiefei mixture can alleviate inflammatory reac-tions and improve clinical symptoms,demonstrating significant efficacy in treating elderly patients with severe pneumonia(syndrome of phlegm-heat obstructing lung).
