清化咳喘汤治疗老年慢性支气管炎急性发作期(痰热郁肺证)的临床观察

摘要
目的 观察清化咳喘汤在老年慢性支气管炎急性发作期(痰热郁肺证)治疗中的效果及对血气指标的影响.方法 90例患者采用随机球号抽取方式分为两组各45例.对照组采用西医常规治疗,研究组在对照组基础上加用清化咳喘汤治疗,比较两组患者治疗效果.结果 研究组总有效率为95.56％,显著高于对照组的82.22％(P＜0.05).研究组治疗后咳嗽、痰量、痰色质、胸满胀满,积分低于对照组(P＜0.05).研究组患者治疗后血清炎性因子水平SAA、C反应蛋白(CRP)、降钙素原(PCT)水平均显著低于对照组(P＜0.05).研究组治疗后血气指标动脉血二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)、酸碱值(pH)水平高于对照组,血氧饱和度(SaO2)水平低于对照组(P＜0.05).结论 清化咳喘汤应用于老年慢性支气管炎急性发作(痰热郁肺证)治疗中临床效果显著,患者经过治疗后炎性因子水平显著降低,血气指标显著改善.
Abstract
Objective:To observe the effect of Qinghua Kechuan Decoction in the treatment of elderly patients with acute exacerbation of chronic bronchitis(syndrome of phlegm-heat stagnated in lungs)and its impact on blood gas indexes.Methods:90 elderly patients admitted were selected.Patients were randomly assigned to a con-trol group(n=45)receiving conventional western medical treatment and a study group(n=45)receiving conven-tional nursing care.The treatment outcomes of both groups were compared.Results:The effective rate of treat-ment in the study group was 95.56％,which was significantly higher than 82.22％in the control group(P＜0.05).After treatment,the scores for cough,sputum volume,sputum characteristics,and chest fullness in the study group were significantly lower than those in the control group(P＜0.05).After treatment,the serum inflammatory factor levels(SAA,CRP,PCT)were significantly lower in the study group compared with the control group(P＜0.05).Af-ter treatment,the blood gas indexes(PaCO2,PaO2,pH)were higher in the study group compared with the control group,while SaO2 levels were lower in the study group compared with the control group(P＜0.05).Conclusion:Qinghua Kechuan Decoction shows significant clinical effects in the treatment of acute exacerbation of chronic bronchitis in elderly patients with syndrome of phlegm-heat stagnated in lungs.After treatment,inflammatory fac-tor levels are significantly reduced,and blood gas indexes are significantly improved.
