中西医结合治疗伏格特-小柳-原田综合征的临床观察

摘要
目的 评价中西医结合治疗伏格特-小柳-原田综合征(VKH)的临床效果.方法 采用病例对照研究方法,将108例(216眼)VKH病例分为中西医结合治疗组(治疗组)56例1 12眼和西医治疗组(对照组)52例104眼,观察治疗前后最佳矫正视力,平均使用糖皮质激素的时间和总剂量、复发率,并进行统计学分析.结果 出院视力＞0.3者治疗组82眼(73.21％),对照组62眼(59.61％),x2=0.451,P＞0.05;平均糖皮质激素使用时间治疗组为(192.6±14.5)天,对照组为(258.4±19.3)天,t=3.845,P＜0.05;使用糖皮质激素总剂量,治疗组为(2 180±76) mg,对照组(3 720±113) mg,t=3.469,P＜O.05.治疗终结后随访2年,观察复发率,治疗组23.21％(26眼),对照组46.15％(48眼),x2=4.954,P＜0.05.结论 中西医结合治疗有利于恢复视力,缩短治疗周期、减少糖皮质激素使用时间和用量,减少并发症的发生,尤其有利于控制该病的复发.
Abstract
OBJECTIVE To evaluate the clinical effect of integrated traditional Chinese and western medicine treating Vogt-Koyanagi-Harada syndrome (VKH).METHODS Using case-control study,108 cases (216 eyes) of VKH patients were divided into integrated treatment group (56 cases,112 eyes),and the western medicine treatment group as control group (52 cases,104 eyes),which were observed and statistical analyzed of the best corrected visual acuity,the average time of using steroid hormone and total dose,recurrence rate before and after treatment.RESULTS There were 82 eyes (73.21％) of discharged patients' visual acuity better than 0.3 in treatment group,and 62 eyes (59.61％) in control group,x2=0.451,P＜0.05; Average time of using glucocorticoid:the treatment group was (192.6 ± 14.5) days,the control group (258.4± 19.3) days,t=3.845,P＜0.05.The total dose of Glucocorticoid hormone:treatment group (2 180±76) mg,the control group (3720±113) mg,t=3.469,P＜0.05.After treatment,following up the patients for 2 years to observe the relapse rate and the treatment group was 23.21％ (26 eyes),46.15％ (48 eyes) in control group,x2=4.954,P＜0.05.CONCLUSIONS The combined treatment of TCM and western medicine was beneficial to the recovery of visual acuity.It could shorten the treatment cycle,reduce using time and dosage of glucocorticoid and reduce the incidence of complications,especially the recurrence of the disease.
