Primary angle-closure glaucoma(PACG)is one of the most common causes of blindness in clinical practice.Poor control of intraocular pressure can lead to optic nerve atrophy,resulting in decreased vision,visual field defects,and ultimately blindness.Western Medicine attributes the disease mainly to anatomical abnormalities or functional impairments in the anterior chamber angle,leading to obstruction of aqueous humor outflow and subsequent elevation of intraocular pressure.Traditional Chinese Medicine,on the other hand,attributes the etiology and pathogenesis of this condition to factors such as liver fire flaming upwards,phlegm-fire stagnation,liver and kidney Yin deficiency,and invasion of pathogenic factors.These factors together cause Qi and blood disharmony,Qi stagnation,and blood stasis,leading to obstruction of the eye's Xuanfu and stagnation of Shenshui resulting in PACG.WEI Qiping specializes in the combined treatment of PACG at different stages of its development,with unique insights into its etiology,pathogenesis,and differential diagnosis and treatment.He advocates treating this condition from the perspective of liver,emphasizing comprehensive systemic treatment alongside intraocular pressure control.He is particularly adept at using a combination of acupuncture and herbal medicine to treat optic atrophy caused by PACG.This article aims to organize and summarize his treatment approach.
