摘要

目的 通过数据挖掘方法分析专利中药复方治疗糖尿病视网膜病变(DR)的用药规律,为临床处方优化及更多的专利开发提供参考依据.方法 选取中国专利公布公告网站(http://epub.cnipa.gov.cn)建库至2023年7月10日中关于中药干预DR的专利.采用R语言中的itemFrequency函数、Apriori算法、hclusts包进行使用频次统计、关联规则分析以及聚类分析,并通过可视化包arulesViz对分析结果进行可视化,最后通过Cytoscape挖掘核心处方.结果 (1)一般情况:共纳入中药专利复方96个,涉及253味中药.(2)使用频次:使用频次>10次的中药(高频中药)有29味,排名前5位的中药为生地黄(54次,21.34%)、三七(44次,17.39%)、黄芪(43次,16.99%)、当归(41次,16.20%)、枸杞子(36次,14.22%).(3)中药属性:四气以寒性(101次,40.00%)最多,五味以甘味(104次,41.12%)最多,归经以肝经(89次,35.17%)最多.(4)中药关联分析:相关性最高的前5位药对是菟丝子-车前子、决明子-茺蔚子、灵芝-蕤仁、密蒙花-黄连、决明子-青葙子.(5)聚类分析:得出5组聚类中药.第I类祛风散热、养肝明目;第II类滋阴清热;第III类补肝益肾明目;第IV类活血化瘀;第V类补气养阴、活血通络.(6)核心中药处方:共挖掘到1个核心处方,即山药、枸杞子、黄芪、生地黄、石斛、甘草、菟丝子、麦冬.结论 中药治疗DR基本治法为益气养阴、活血通络,多选用活血化瘀及补虚类中药,根据兼症不同选用不同的中药配合治疗,新处方可用于疾病各期.

Abstract

OBJECTIVE To analyze the medication patterns in patented Traditional Chinese Medicine(TCM)formulas for the treatment of diabetic retinopathy(DR)using data mining methods,providing reference for optimizing clinical prescriptions and developing new patents.METHODS Patents related to TCM interventions for DR were selected from the China Patent Publication Announcement Database of the National Intellectual Property Administration up to July 10th,2023.The item Frequency function,Apriori algorithm,and hclust package in R language were used for frequency statistics,association rule analysis,and cluster analysis,respectively The results were visualized using the arulesViz package,and core prescriptions were identified using Cytoscape.RESULTS(1)General overview:A total of 96 patented TCM formulas,involving 253 Chinese medicines,were included.(2)Frequency of use:There were 29 Chinese medicines with a usage frequency of more than 10 times(high-frequency Chinese medicines),with the top five Chinese medicines being Rehmanniae Radix(54 times,21.34%),Notoginseng Radix(44 times,17.39%),Astragali Radix(43 times,16.99%),Angelicae Sinensis Radix(41 times,16.20%),and Lycii Fructus(36 times,14.22%).(3)Chinese medicines properties:In terms of the four Qi,cold(101 times,40.00%)was the most common.Among the five tastes,sweet(104 times,41.12%)was the most common.In terms of meridian tropism,liver meridian(89 times,35.17%)was the most common.(4)Chinese medicines association analysis:The top five Chinese medicines pairs with the highest correlations were Cuscutae Semen-Plantaginis Semen,Cassiae Semen-Leonuri Semen,Ganoderma-Semen Torreyae,Buddlejae Flos-Coptidis Rhizoma,and Cassiae Semen-Celosiae Semen.(5)Cluster analysis:Five clusters of Chinese medicines were identified.Cluster I:Disperses wind and clearing heat,nourishing liver,and improving vision.Cluster II,Nourishing Yin and clearing heat.Cluster III,Tonifing liver and kidney,and improves vision.Cluster IV,Invigoratingblood and removing stasis.Cluster V,Tonifing Qi and Yin,invigorating blood,and unblocking the collaterals.(6)Core TCM prescription:One core prescription was identified,including Dioscoreae Rhizoma,Lycii Fructus,Astragali Radix,Rehmanniae Radix,Dendrobii Caulis,Glycyrrhizae Radix,Cuscutae Semen,and Ophiopogonis Radix.CONCLUSIONS The basic treatment principles for DR with TCM are to tonify Qi and Yin,invigorate blood,and unblock collaterals.Blood-invigorating and deficiency-tonifying Chinese medicines are frequently used,with different Chinese medicines selected for various concomitant symptoms.The new prescription can be applied to different stages of the disease.
