摘要
本文主要基于"一气周流,土枢四象"理论与临床实践的结合,探讨"一气周流,土枢四象"在黄斑水肿临床上的应用.通过整理医案,阅读相关古籍、文献,深入探讨"一气周流,土枢四象"理论在黄斑水肿的发生、发展以及治疗中的应用.笔者团队认为黄斑水肿的发生,究其源头,当责之于脾胃中气枢纽的异常,脾胃枢纽异常,则肝、心、肺、肾四象运转不利,并在此过程中出现痰、湿、瘀等病理产物,进一步影响中土、四象的正常功能.故而在治疗上,提出了以顾护中土为基础,重视调护四象气机升降的流畅,并兼顾祛除病理产物的思路,取得了较好的临床疗效.故本文通过总结临床经验,并基于"一气周流,土枢四象"理论对黄斑水肿的病机、治疗进行讨论,以期为临床治疗眼底病提供更多思路.
Abstract
This paper explores the clinical application of the"Qi circulation and earth as pivot for the four elements"theory in the treatment of macular edema,based on the integration of this theory with clinical practice.By reviewing medical cases,reading relevant classical texts and literature,we delve into the role of this theory in the occurrence,development,and treatment of macular edema.Our team believes that the onset of macular edema can be traced back to an abnormality in the pivotal function of the spleen and stomach.When this central pivot is disturbed,the normal functioning of liver,heart,lung,and kidney is impaired.This dysfunction leads to the formation of pathological products such as phlegm,dampness,and stasis,further affecting the normal operations of the central earth and the four elements.Therefore,our treatment strategy is based on protecting the central earth,emphasizing the smooth flow of the Qi mechanisms of the four elements,while also addressing the removal of pathological products.This approach has yielded favorable clinical outcomes.This paper summarizes our team's clinical experiences and discusses the pathogenesis and treatment of macular edema based on the"Qi circulation and earth as pivot for the four elements"theory,aiming to provide additional insights for the clinical treatment of retinal diseases.
