Exploring the Application of Buyang Huanwu Decoction with Modification Based on the State-targeted Syndrome Differentiation in Qi Deficiency and Blood Stasis Type Dia-betic Retinopathy
As living standards improve, diabetes has become a common and prevalent disease clinically. If left untreated,it can lead to a series of complications, with diabetic retinopathy(DR)being one of the most common.Pathologically characterized by retinal microaneurysms, hemorrhages, exudates, neovascularization, and fibrous tissue proliferation, the main clinical treatment methods currently focus on controlling risk factors such as blood sugar, blood pressure, and blood lipids, along with surgeries, medication injections, and laser treatments when necessary, albeit with limitations.Traditional Chinese Medicine(TCM)demonstrates significant advantages in treating DR.However, relying solely on the TCM method of"syndrome differentiation and treatment"may be somewhat inadequate in treating asymptomatic patients. Therefore,Academician TONG Xiaolin, through extensive clinical practice, has integrated TCM syndrome differentiation thinking with modern medicine, establishing the"state-target-causality"syndrome differentiation strategy, which has shown definite efficacy in treating diabetes and its complications.Based on their experience in treating DR,the team believes that the disease is mainly rooted in Qi deficiency with blood stasis in the collaterals.Treatment principles emphasize"tonifying Qi,activating blood circulation, and dredging collaterals,"with the use of Buyang Huanwu Decoction to regulate the state while targeting the causative factors.Precise targeting of retinal changes using a combination of Coptis chinensis,Panax notoginseng, and Lycium barbarum has shown definite clinical efficacy.
