Treatment of Primary Open-angle Glaucoma from the Perspective of"State Target Differentiation and Treatment"
Based on the"pattern-target diagnosis and treatment"system in Traditional Chinese Medicine(TCM)proposed by academician TONG Xiaolin,this study explores the application of the"pattern-target"theory to the modern diagnosis and treatment of primary open-angle glaucoma(POAG),following the staging criteria in contemporary medicine.Guided by TCM principles,this paper summarizes the core pathogenesis of glaucoma at different stages:Early-stage characterized by liver Qi stagnation and disharmony of Qi and blood;Progression stage characterized by liver depression,spleen deficiency,and blood stasis obstructing the collaterals;And Late-stage characterized by liver and kidney deficiency with insufficiency of essence and blood.We propose a pattern of change termed as"stagnation pattern-stasis pattern-deficiency pattern"for these stages.Corresponding to the pathogenesis,main symptoms,and laboratory indicators at each stage,targeted prescriptions and medicines are selected,achieving pattern-target harmonization.This approach constructs a"pattern-target"strategy for the diagnosis and treatment of POAG,aiming to enhance the specificity and precision of clinical therapy.
