目的 系统评价疏肝类方药治疗干眼的临床疗效.方法 检索PubMed、Embase、Web of Science、中国知网、万方、维普及中国生物医学文献数据库中关于疏肝类方药治疗干眼的随机对照试验(RCT)文献,检索时间为建库至2024年3月31日.对文献按照纳入标准与排除标准进行筛选和数据录取,Cochrane偏倚风险评估工具对纳入文献进行质量评价,采用RevMan 5.4和Stata 17.0软件对纳入文献的临床总有效率、中医证候评分、泪液分泌量、泪膜破裂时间作为主要结局指标进行Meta分析.结果 共纳入17篇文献,总样本量1,593例,其中观察组805例,对照组788例.疏肝类方药联合西药在提高总有效率[RR=1.290,95%CI(1.150,1.460),Z=4.210,P=0.000]、降低中医证候评分[MD=-1.860,95%CI(-3.110,-0.620),Z=2.930,P=0.003]、增加泪液分泌量[MD=2.250,95%CI(1.460,3.040),Z=5.610,P=0.000]、延长泪膜破裂时间[MD=2.630,95%CI(1.610,3.660),Z=5.020,P=0.000]方面均优于单纯西药治疗,差异有统计学意义.2组角膜荧光染色评分比较,差异无统计学意义(P＞0.05).结论 疏肝类方药能够提高干眼患者的治疗总有效率,降低中医证候评分,增加泪液分泌量,延长泪膜破裂时间.
Meta Analysis of the Clinical Efficacy of Liver-soothing Formulas in the Treatment of Dry Eye
OBJECTIVE To systematically evaluate the clinical efficacy of liver-soothing formulas in the treatment of dry eye.METHODS Randomized controlled trial(RCT)on the use of liver-soothing formulas for dry eye were retrieved from PubMed,Embase,Web of Science,CNKI,Wanfang,VIP,and CBM databases,with a search period from database inception to March 31st,2024.Literature was screened according to inclusion and exclusion criteria,and data were extracted.The Cochrane Risk of Bias Tool was used to assess the quality of the included studies.RevMan 5.4 and Stata 17.0 software were utilized to perform a meta-analysis on primary outcome indicators,including total clinical efficacy rate,TCM syndrome score,tear secretion,and tear film break up time(BUT).RESULTS A total of 17 studies involving 1,593 cases were included,with 805 cases in the experimental group and 788 in the control group.Liver-soothing formulas combined with Western Medicine(WM)demonstrated superior outcomes compared to WM alone,with improvements in total efficacy rate[RR=1.290,95%CI(1.150,1.460),Z=4.210,P=0.000],reduction in TCM syndrome score[MD=-1.860,95%CI(-3.110,-0.620),Z=2.930,P=0.003],increase in tear secretion[MD=2.250,95%CI(1.460,3.040),Z=5.610,P=0.000],and prolonged tear film breakup time[MD=2.630,95%CI(1.610,3.660),Z=5.020,P=0.000],all differences were statistically significant.There was no statistically significant difference between the two groups in corneal fluorescein staining score(P>0.05).CONCLUSIONS Liver-soothing formulas can improve the total treatment efficacy,reduce TCM syndrome scores,increase tear secretion,and prolong BUT in patients with dry eye.
