基于主成分分析和聚类分析的中央型气道恶性肿瘤中医证候研究

目的 分析中央型气道恶性肿瘤的中医证候特点,探索其中医证型分布规律.方法 收集298例中央型气道恶性肿瘤患者的中医四诊信息,运用主成分分析法,结合证候要素判定标准,确定此病中医证候要素特点;再应用系统聚类分析法,对条目进行聚类分析,探索此病中医证候分布规律.结果 中央型气道恶性肿瘤的证候要素主要为痰浊、气虚、痰热、血瘀和湿热.聚类分析结果显示,其中医分型以痰热初起证占比(35.6%)最多,其余依次为痰热兼有肺肾气虚证(33.6%)、脾肾阳虚证(5.3%)、寒湿兼有瘀血证(4.0%).结论 中央型气道恶性肿瘤的病位主要在肺,病性属虚实夹杂、本虚标实,临床辨证可分为痰热初起证、痰热兼有肺肾气虚证、肺气阴两虚证、脾肾阳虚证、寒湿兼有瘀血证,其中以痰热初起证、痰热兼有肺肾气虚证占比较高.
Study on the Traditional Chinese Medicine Syndrome of Central Airway Malignant Tumor Based on Principal Component Analysis and Cluster Analysis
Objective To analyze the characteristics of TCM syndrome of central airway malignant tumor and explore the distribution pattern of TCM syndrome types.Methods The information of four TCM consultations of 298 patients with central airway malignant tu-mor was collected,and the characteristics of TCM syndrome elements of central airway malignant tumor were determined by using principal component analysis,combined with the criteria for determining evidence elements,and then the entries were clustered by ap-plying systematic cluster analysis to explore the distribution pattern of TCM syndrome patterns of central airway malignant tumor.Re-sults The evidence elements of central airway malignant tumor were mainly phlegm turbidity,qi deficiency,phlegm heat,blood stasis and damp heat.The results of cluster analysis showed that the TCM typology of central airway malignant tumor was dominated by ini-tial syndrome of phlegm heat(35.6%),with the rest being phlegm heat with deficiency of lung and kidney qi(33.6%),syndrome of yang deficiency of spleen and kidney(5.3%),and cold dampness with blood stasis syndrome(4.0%).Conclusion Central airway malig-nant tumor is mainly located in the lung,which is intermingled deficiency and excess syndrome,and asthenia in origin and asthenia in superficiality syndrome.They can be clinically classified as deficiency of lung and kidney qi,phlegm heat with deficiency of lung and kidney qi,cold dampness with blood stasis syndrome,syndrome of yang deficiency of spleen and kidney.
