摘要
肱骨髁上骨折是儿童上肢骨折中最常见的骨折类型.在外力、肌肉和骨膜的共同作用下,骨折块不仅会发生轴向移位,还可能伴随着一定程度的旋转移位.快速、准确地评估旋转移位对于术中复位、选择固定方式以及预测术后功能具有重要意义.目前用于评估旋转移位的方法包括X线片、CT和超声,但各种评估方法和标准较易混淆.本文回顾和整理了当前基于各种影像学评估旋转移位的方法,将复杂、立体的空间旋转转化为冠状面、矢状面及水平面的单平面旋转,以期为临床应用提供参考.
Abstract
Supracondylar humerus fractures represent the most prevalent type of upper limb fractures in pediatric patients.As a result of the interplay of external forces,muscular contractions,and periosteal reactions,these fractures are prone not only to axial displacement but also to rotational displacement in a certain degree.Rapid and accurate assessment of the rotational displacement holds a paramount importance for intraoperative re-alignment,choice of fixation modalities,and anticipation of postoperative function.Currently,methods to evalu-ate the rotational displacement involve X-ray,CT,and ultrasound,but the various assessment techniques and cri-teria often pose challenges in differentiation.This paper reviews the current diverse imaging modalities for assess-ment of the rotational displacement,aiming to translate the intricate three-dimensional spatial rotation into sin-gle-plane rotations within the coronal,sagittal,and horizontal planes,thereby aspiring to furnish those concerned with clinical guidance.
