Objective To investigate the clinical characteristics and laboratory test in patients with calcium pyrophosphate deposition(CPPD).Methods The clinical data,imaging features,ultrasonic features,pathological features and laboratory test results of 20 cases of calcium pyrophosphate deposition disease were collected.SPSS 22.0 software was used for analysis.The non-normal distribution of econometric data was represented by[M(Q1,Q3)],and comparied by Man-Whitney U test.The counting data was represented by count(%),and comparied by chi-quare test.Results The 20 CPPD patients included 7 males and 13 females,with a male to female ratio of 1∶1.9 and the median age was 68.5 years,The clinical manifestations were diverse,mainly arthritis.All 20 CPPD patients had symptoms of arthritis,and the imaging showed varying degrees of damage;and bone erosion in 7 cases.One routine histopathological examination revealed lumpy and multifocal deposition of purple blue calcified crystals in synovial tissue.Strip,rod and rhomboid calcium pyrophosphate crystals were detected in the synovial fluid of 20 CPPD patients;The level of white blood cell count in synovial fluid was higher than that in blood,and the difference was statistically significant[38.0(6.1,70.3)×109/L vs.8.8(7.3,11.9)×109/L,Z=-2.49,P=0.013].The neutrophil/lymphocyte ratio level of synovial fluid was higher than that of blood,and the difference was statistically significant[21.5(10.4,42.4)vs.4.1(2.9,7.7),Z=-3.25,P=0.001].The level of total protein in synovial fluid was lower than that in blood,and the difference was statistically significant[35.8(31.3,39.6)g/L vs.67.4(62.2,70.1)g/L,Z=-4.30,P=0.001].Conclusion Calcium pyrophosphate deposition disease usually occurs in middle-aged and elderly people,mostly in females,with diverse clinical manifestations.The gold diagnostic standard is calcium pyrophosphate crystals in the synovial fluid of the affected joints,or calcium pyrophosphate crystals in biopsy specimens.
