Effects of glucocorticoids and cyclophosphamide and hydroxychloroquine sulfate on resting brain function in systemic lupus erythematosus patients without major neuropsychiatric manifestations

目的 通过分析静息态功能磁共振数据,探究糖皮质激素(GC)、GC联合环磷酰胺(CTX)和/或硫酸羟氯喹(HCQ)治疗对无明显神经精神表现系统性红斑狼疮患者脑功能的影响,为早期识别和干预无明显神经精神表现SLE患者脑功能的变化提供依据.方法 根据入组时的治疗情况将无明显神经精神表现SLE患者分为初发未治疗组、单用GC组、GC联用CTX和/或HCQ组,并选择性别、年龄、受教育年限匹配的健康对照(HCs),计算各被试的低频振幅(ALFF)、比率低频振幅(fALFF)和局部一致性(ReHo).多组间比较时,计数资料采用x2检验或Fisher精确检验,连续变量采用方差分析或Kruskal-Wallis H检验,事后比较采用LSD-t法.结果 4组匹配后分别入选22例、30例、50例和29例,4组间多个脑区的脑功能差异有统计学意义.具体而言,相较于HCs组,初发未治疗组右侧中央后回(t=-2.96)、右侧中央旁小叶(t=-2.95)的ALFF值降低,GC组在右侧中央旁小叶(t=-2.93)、右侧中央后回(t=-2.93)、右侧中央前回(t=-2.94)的ALFF值降低,GC+CTX和/或HCQ组右侧中央旁小叶(t=-2.90)、右侧中央前回(t=-2.90)的ALFF值降低;与初发未治疗组相比,GC+CTX和/或HCQ组的ALFF值在右侧中央后回(t=-3.00)、右侧中央沟盖(t=-2.91)升高.相较于HCs组,初发未治疗组右侧中央前回(t=-2.95)、左侧中央后回(t=-2.97)的fALFF值降低,GC组右侧中央前回(t=-2.94),左侧中央后回(t=-2.98)的fALFF值降低,GC+CTX和/或HCQ组左侧中央后回(t=-2.91)的fALFF值降低;GC+CTX和/或HCQ组的fALFF值在右侧中央前回(t=-2.89)较GC组升高.与HCs比较,初发未治疗组在右侧中央后回(t=-2.97)的ReHo值降低,GC组在右侧中央前回(t=-2.95)的ReHo值降低,GC+CTX和/或HCQ组的ReHo值在右侧中央前回(t=-2.91)较初发未治疗组升高,右侧中央后回(t=-2.93)的ReHo值较GC组升高.以上结果均采用高斯随机场(GRF)进行多重比较校正(Pvoxel<0.005,Pcluster<0.010).结论 无明显神经精神表现SLE患者在全脑多个脑区功能活动降低,使用GC联合CTX和/或HCQ较单用GC在改善脑功能方面可能有更好的效果,并且主要作用于感觉运动相关的脑区.
Objective By analyzing resting state functional magnetic resonance imaging data,this study aims to explore the effects of glucocorticoids(GC),glucocorticoids combined with cyclophosphamide(CTX)and/or hydroxychloroquine sulfate(HCQ)treatment on brain function in systemic lupus erythematosus patients without major neuropsychiatric manifestations,providing a basis for early identification and intervention of changes in brain function in systemic lupus erythematosus patients without major neuropsychiatric manifestations.Methods Systemic lupus erythematosus patients without major neuropsychiatric manifestations were divided into drug naïve group,GC group,and GC combined with CTX and/or HCQ group based on their treatment status at the time of enrollment.Health controls(HCs)were selected based on gender,age,and education level.The low frequency amplitude(ALFF),fractional low frequency amplitude(fALFF),and regional homogeneity(ReHo)were calculated for each of the four groups of participants.When comparing multiple groups,x2 test or Fisher's exact test was used for count data,analysis of variance or Kruskal-Wallis H test was used for continuous variables,and LSD method was used for post hoc comparisons.Results After matching,22,30,50,and 29 cases were finally included.There were significant differences in brain function in multiple brain regions among the four groups.Specifically,compared with the HCs group,the ALFF values of the right posterior central gyrus(t=-2.96)and the right paracentral lobule(t=-2.95)in the drug naïve group were decreased.In the GC group,the ALFF values in the right paracentral lobule(t=-2.93),the right posterior central gyrus(t=-2.93)and the right anterior central gyrus(t=-2.94)were decreased.The ALFF values of the right paracentral lobule(t=-2.90)and the right anterior central gyrus(t=-2.90)were decreased in GC+CTX and/or HCQ groups.Compared with the drug naïve group,ALFF values were higher in the right posterior central gyrus(t=-3.00)and the right rolandic operculum(t=-2.91)in GC+CTX and/or HCQ groups.Compared with the HCs group,fALFF values of the right anterior central gyrus(t=-2.95)and the left posterior central gyrus(t=-2.97)in the drug naïve group decreased,and fALFF values of the right anterior central gyrus(t=-2.94)and the left posterior central gyrus(t=-2.98)in the GC group decreased.fALFF values decreased in the left posterior central gyrus(t=-2.91)of GC+CTX and/or HCQ groups.fALFF values in the right anterior central gyrus(t=-2.89)were higher in GC+CTX and/or HCQ group than in GC group.Compared with HCs,the ReHo value in the right posterior central gyrus(t=-2.97)of the drug naïve group decreased,and the ReHo value in the right anterior central gyrus(t=-2.95)of the GC group decreased.The ReHo of GC+CTX and/or HCQ group was higher in the right anterior central gyrus(t=-2.91)than that of the drug naïve group,and the ReHo of the right posterior central gyrus(t--2.93)was higher than that of the GC group.The above results were adjusted by Gaussian random-field(GRF)multiple comparison(Pvoxel<0.005,Pcluste<0.010).Conclusion Systemic lupus erythematosus patients without major neuropsychiatric manifestations have reduced functional activity in multiple brain regions of the whole brain.GC combined with CTX and/or HCQ may have better effects on improving brain function than GC alone,and it mainly acts on sensorimotor related brain regions.
